Dear Mr. Madsen:

The following information regarding violations cited by the USNRC Inspectors in IE Inspection Report No. 50-382/ Report 82-08 dated June 17, 1982, is here-with submitted.

Failure to follow procedures / instructions for the control of temporary and as-built pipe supports / restraints l

Criterion V of 10 CFR 50, Appendix B, requires that activities af fecting safety be performed in accordance with the prescribed procedures or instruc-tions.

I The NRC Inspectors identified the following conditions where procedures involving safety-related activities were not followed:

A.

Condition identification work authorization (CIWA) Number 823607, dated March 29, 1982, required removal of temporary hanger SIRR-500lT, Rev. O, from the north pump room and installation of SIRR-500lT, Rev. 2, in the south pump room. The closed CIWA indicated work completed on April 2, 1982.

B.

The requirement of Ebasco Procedure ASP-IV-89, Issue A, is "To assure pipe is adequately supported for hydrostatic testing and flush".

Contrary to the above, the NRC Inspector observed pipe spool 2803-IC-35 supported by a wooden wedge in blockout rather than by required hanger SIRR-183, even though the system walkdown for flush and hydro was complete.

Paragraph 6.5.6 of ASP-IV-89,. Issue A; states:

"The Construction Engineer Mechanical (CEM) will inspect the temporary support for conformance to the temporary design drawing. He will sign and date the temporary request form (attachment 7.2) indicating the temporary support is in-place per the design..."

Contrary to the above, the as-built condition of temporary hanger SISR-8707 (System 60A) devia tes f rom design although signed and dated attachment 7.2 indicates compliance.

Corrective Action Taken and Results Achieved i

Temporary Hanger SIRR-5001T, Rev. O is presently located in North Pump Room as observed by the NRC Inspector. CIWA 825629 has been issued directing removal of temporary hanger SIRR-5001T, Rev. O.

CIWA 823607 which was inadvertantly closed, is to be recalled by Ebasco for reevaluation and disposition.

Corrective Action Taken to Preclude Repetition a.

The temporary hanger log, as maintained by ESSE, is being reviewed to provide a current status of all temporary hangers, b.

Procedure ASP-IV-89 has been revised to include consideration of piping vibrations experienced during hydro / flush testing by providing LP&L with the allowable span vibrational amplitudes before the test and having LP&L submit vibration measurements to ESSE via a CIWA for evaluation, should they exceed the precalculated values. The disposition must be accomplished before test continuation.

In addition, ASP-IV-89 paragraph 6.1.5.7 will be revised to require LP&L piping verification group to Independently verify the installation.

c.

Return of the Form 7.3 " Support / Restraint Location Acceptance for Hydro /

Flush Operations" back to ESSE until problem resolution is established and implemented.

d.

If additional temporary support is required, ESSE will design and release the sketch to CEM.

e.

CEM or his designee will inspect the temporary support installation and sign and date the temporary request Form 7.2.

Copies of this form are sent to LP&L Piping Verification Group in order that they may independently verify the installation.

Upon notification f rom CEM of the new temporary support installation, ESSE will re-release revised Form 7.3 to permit continuation of hydro / flush testing.

Ebasco Memo MRS-SU-6041 issued 7/21/82 emphasizing necessity for properly processing CIWA.

B.

Co_rrective Action Taken and Results Achieved Hanger SIRR-183 was field inspected by Ebasco Construction Engineering and found to be completely installed and erected in accordance with the applicable design sketch. Documentation indicates hanger completed by Contractor on March 6, 1982 and released for hydro.

The wooden wedge had been used as a support during the rework of a weld on the opposing side of a wall penetration, which necessitated the removal and reinstallation of hangers. Wooden wedge has subsequently been removed.

Q.C. inspection subsequent to completion of the rework and hanger reinstal-lation did not occur; as a consequence the file for hanger SIRR-183 was incomplete, llangers in question will be reinspected.

Corrective Action Taken to Preclude Repetition Corrective steps have been taken to adequately control the process by which support systems are reviewed, inspected and accepted prior to any actual loading during hydrostatic tests or hydro. Ebasco Procedure ASP-IV-89, Rev. B, establishes specific controls with regards to how supports are determined to be required, inspected and approved.

Implementation of the procedures and required interface and compliance by Contractor precludes recurrence.

Date When Full Corrective Action Will Be Achieved Completion of corrective action will occur by August 9,1982.

C.

Corrective Action Taken and Results Achieved:

Completion of the permanent installation of Support SISR-870 to the latest revision and the removal of the structural spacer has been accomplished.

ESSE to complete and release Form 7.3 upon notification'of the status by Construc tion Engineer Mechanical ( CDl).

Corrective Action Taken to Preclude Repetition Ebasco procedure ASP-IV-89 has been revised to require CEM, or his designee, verif y the installation of the temporary piping and notify LP&L Piping Verifi-cation Group to conduct an independent check of the status.

Date When Full Corrective Action Will Be Achieved Full corrective action will be complete by August 10, 1982.
