Attention:

Mr. James G. Keppler, (k

Director RE:

WM. H. ZIMMER NUCLEAR POWER STATION - UNIT 1 10CFR50.55(e) S-1, CONNECTION FAILURE OF THE INTAKE FLUME TO THE SERVICE WATER PUMP STRUCTURE, DOCKET NO. 50-358, CONSTRUCTION PERMIT CPPR-83, W.O. #57300, JOB E-5590, FILE NRC-9 & #3200 Gentlemen, This is an interim progress report to inform you of our progress concerning the failure of the weld at the embedded plate and sheet piling waler connections of the intake flume to the service water pump structure, a condition reportable under 10CFR50.55(e).

Mr. Paul Barrett of Region III, was verbally informed of this condition on January 7, 1981, while he was at the Zimmer Site conducting an inspection.

Subsequently, followup information was provided to the NRC Region III in written reports on January 12, 1981, and on February 2, 1981.

Our Architect Engineer ha's investigated the weld failure and determined the cause and resolution of the condition.

The as-built weld connection of the horizontal sheet piling walers at elevation 506' Sb" to the embedded plates on the service water pump structure were not i stalled per the design drawings or shop drawings.

provided a one-sided weld clip angle connec-The installed connect.

+

tion instead of.a concentric two-aided connection as detailed on the drawings..Since the monnection transfers both shear and tensile loads, an eccentricity was created during any lateral loading of the sheet piling.

' Attn:

'J. G. Keppler', Director QA-1526 10CFR50.55(e) S-1 Page-2 The construction tolerances in driving the. sheet piling necessitated the waler on the south ' side to be installed with' slight offset from.the center of.the embedment plate.'

This' offset created additional eccentricity-on the connecting joint.

Further investigation by our Quality Ascurance Department indicate similar irregularities at the intermediate ani lower elevation connections also exist (Elev. 478' and 458' respectively).

It is the opinion'of our Architect-Engineer that the aforementioned connection irregularities have degraded the r.onnection resulting in weld failures.

Presently, thIe lower.. intermediate and upper elevation connections on the south side of the flumehave been~ corrected and inspected. The lower elevation'on the north side has also been corrected. Work is in progress on the intermediate and upper elevation (Elev. 478'.and 605' respectively) and will be complete in six to eight weeks.

We will keep you informed on our progress as it develops.

Very truly yours, THE CINCINNATI GAS & ELECTRIC COMPANY By B. R SYLVIA VICE-PRESIDENT, NUCLEAR OPERATIONS' EAB/NB/ejc cc: NRC Resident Inspector Attn:
