In response to your letter of November 9, 1981, we are hereby submitting this amended response to Appendix A, Violation D of the subject Inspection Report.

4 You _ stated that.our original response, dated October 15, 1981, to this Violation did not fully address actions taken to correct specifically identified documentation deficiencies including correction of Onsite Safety Committee minutes and elimination of the existing backlog of unissued meeting minutes. The additional actions included in this amended response address these concerns.

Agended Reply to' Notice of Violation Appendix A. Violation D Inspection No. 81-18

' Letter dated September. 14,.1981-VIOLATION D (Severity' Level VI; Supplement I)

Description of Violation (81-18-08)

Technical Specification 6.5.1.8 states "The OSC shall maintain written minutes of each meeting and copies shall be provided to the General Superintendent of -

Power Stations and Chairman of the Of fsite Review Committee."

- Beaver Valley Power Station Administrative Procedure, Chapter 10, Onsite Safety Committee (OSC), Revision 1, September 26, 1980; Step VI.A.2 states, " Minutes shall be recorded for all meetings, and all significant Committee actions shall be documented; minutes should be distributed to Committee members for review within one month of the -meeting."

Contrary to the above, the following Committee actions were not docum6nted in OSC meeting minutes. generated during 1980:

Fourteen instances-in which the quorum requirements at meetings were not clearly documented.

One instance in which a procedure review was not documented.

Review of sixteen out of 200 temporary procedure changes were not documented.

I Investigations of violations identified in three ' inspections were not documented.

Many instances in which meeting minutes were not distributed for review within one month.

Corrective Action Taken Item No. 1:

The Onsite Safety Committee (OSC) will review those meeting minutes where the established quorum was considered to be inadequately documented.

Item No. 2 The specifically identified instances of inadequate documentation and 3:

of OSC review will be addressed in a future OSC meeting to provide documentation to support the review of these items.

In addition, a check of OSC reviews for the current issue of Operating Manual Chapters will be conducted to assure the OSC review has been properly documented, j

~ Description of Violation Item No. 4:

As stated in this inspection-report, all inspection reports were-documented in the OSC meeting minutes.

It is agreed that the tmethod of documenting committee actions may not be consistent,-

however, each inspection report was reviewed and the. statement"

...for information only" documents that the inspection report was presented to the OSC.

The meeting minutes do state that all items on the agenda were reviewed.

Item No.'5:

Additional personnel have been assigned to reduce the backlog of unissued OSC meeting ' minutes.

Action Taken To Prevent Recurrence Item No.1:

Alternates attending the OSC meeting-are more clearly documented to assure quorum requirements are met at the beginning of the meeting. It should be noted that this violation concerned Onsite Safety Committee (OSC) meetings conducted in 1980.

Technical Specification (TS) Amendment No. 39, issued on February 11, 1981, reconstituted the OSC membership.

Since this change, the quorum of the OSC has been properly established and documented for OSC meetings.

Item No. 2 The members of the OSC have been instructed to provide the OSC

.and 3:

Recording Secretary with information on materials presented for OSC review so that the meeting minutes will properly document these activities.

Item No. 4:

To - avoid confusion in the future, the OSC Recording Secretary will be required to be more specific in documenting the reviews conducted by the OSC.

Item No. 5:

Hand written meeting minutes will be distributed to meeting ettendees to insure a timely review within the one month time frame specified in the Beaver Valley Power Station Administrative Procedures.
