O-Attachment.I Violation Technical Specification 6.8.1 requires that written pro-cedures be implemented covering the applicable procedures re-commended in Appendix "A" of Regulatory Guide 1.33, November, 1982. Regulatory Guide 1.33, November,1972, G.2, Solid Waste System, lists baling machine operation as a typical safety related activity-that should be covered by a written procedure.
SOP-WDS-5, " Baling Machine Operation," Step 4.3 states in part,
....see that the drum is centered under the plate, close the shroud door.
' Contrary to the above, on May 27, 1982, solid radioactive waste 1 operations were. conducted, including the use of a baling machine, without the use of a procedure, and during operation, the shroud door was not closed as required.
Response
Personal safety must be foremost in the minds of all personnel employed by the Power Authority.
When questioned, the personnel responsible for radioactive waste processing admitted operating the baling machine with the shroud door-open,-but only to increase the compaction' efficiency and not because of convenience.
Unfortunately that function was conducted at the expense of their personal _ safety.
Upon notification of the non-compliance by your inspector, l
the practice.of baling with the"shr.oud door open was immediately stopped. All personnel that'could be assigned that function were
~
reinstructed in the proper method of baling machine operation and
'in the safety consequences of procedural violations.
SOP-WDS-5_has been canceled and baling operations are now included in a more. appropriate procedure, HPI-9.20 " Radioactive
-Waste Handling and Packaging".
