1/5/81 Identification of Occurrence:
Reactor core isolation cooling subsystem inoperable.
Conditions Prior to Occurrence:
l Unit 2 at 1004 power.
I Description of Occurrence:
i During arercillance testing RCIC pump floor indication was observed upscale with the paup secured.
Investigation revealed a failed square root extractor in l
the flow control loop.
l Analysis of Oecurrence:
Safety iglications are considered minimal. The HPCI subsystem was immediately tested and proven operable.
The RCIC subsystem was scill operable in manual from the control room and ins 11y operable from the emergency shotdown panel.
Corswetive Action:
'.'hdodimetive sepmas root extractor was replaced. The RCIC subsystem was
,-ammassaftally tested and returned to service.
