This report is sutmitied pursuant to 10CFR50.55(e). It describes Seismic I hangers installed by Torrpkins-Beckwith, Inc. which do not conply with design and installation drawings nor with MS requirements. The hangers were among those inspected and accepted by the Quality Control department of

> Tortpkins-Beckwith and were included in turnover packages for start-up systems released to the Ovner. This reflects a breakdown in the Quality Control / Qual i ty Assurance Programs of Tonpkins-Beckwi th, Inc. , Ebasco Services, Inc. and Louisiana Power and Light Conpany. This condition is considered reportable under the requirements of 10CFR50.55(e). To the best of our knowledge, this problem has not been reported to the Nuclear Regulatory Corrmission pursuant to 10CFR21.

DESCRIPTION:

Upon receipt of turnover documents for Start-Up System 60B - Low Pressure Safety injection, Louisiana Power S Light (LPSL) Q4 perfonned an audit of docunentation for hanger installations contained in the turnover package.

LP&L Q4 identifled nurnerous discrepancies which included: installation that was not in accordance with design docunents, welds called for on the design docunents/ detail drawings that do not meet AVS size requironents, field welds that were not made per requirements between the as-built drawings and actual construction. This condition indicates a breakdown of the Quality Assurance programs of Tonpkins-Beckwi th, Inc. , Piping Contractor, Ebasco Services, Inc. and Louisiana Power & Light C7npany in that the Tccpkins-Beckwith (T-B) turnover packages that were sutmitted contained discrepancies between the final Quality Assurance / Quality Control certification and the final actual construction.

SAFETY EVALLMTION:

The deficiencies frun accepted as-built hanger sketches as described herein occur on Seisnic Category I systems required to mitigate the consequences of an accident and bring the plant to a safe shutdown condition. Untii the extent of the deficiencies are known, it nust be assuned that they could detract fran the ability of these systems to perform their function under SSE condi tions.

GJRRECTIVE ETICN Corrective Action has been irrplemented as follavs:

(1) T-B has carpleted a retraining program with particular enphasis on as-buiit records for alI Hanger Engineering personneI and QuaIity Control inspectors to requalify them to properly perform the field inspections of pipe support / restraint and to carplete the necessary documentation.

(2) T-B Quality Control and Engineering have initiated a reinspection of approximately 4400 hangers which were cortpleted and accepted prior to July 6, 1982. NCR W3-4010 will be supplanented to track the disposition and '

corrective action taken for hangers found to have deviations from design requirements which were corrpleted and accepted prior to July 6,1982. Any rework required will be noted on NCR W3-4010 which shall then be returned to T-B for their action.

(3) Presently speci fied fillet weld sizes meet structural design requirenents.

Havever, in order to verify the adequacy of the specifled weld size, T-B shall quali fy existing prequalified AhS fillet weld procedures to verify the acceptabiiity of performing trultipass fiilet weIds using smalI diameter esectrodes. Rese qualified procedures wili verify the acceptability of specifying and welding fillet weld sizes smaller than AWS allowables.

(4) A Torrpkins-Beckwith Quality Assurance Audit is undenvay to inspect a sartple lot of hangers to veri fy that the reinspection and future inspections are perfonned properly.

(5) Ebasco Services incorporated has carpleted a retraining program for Quality Assurance personnel involved in the review of piping hanger docurentat ion.

Corrective action will be carpleted by February 15, 1983 and a Final Report will be subnitted to the PRC at that time.
