SEQUOYAH NUCLEAR PLANT - RESPONSE TO IE BULLETIN NO. 82-02: DEGRADATION OF THREADED FASTENERS IN THE REACTOR COOLANT PRESSURE BOUNDARY OF PWR PLANTS In response to your letter dated June 2, 1982, enclosed is the response to action item 3 of IE Bulletin 82-02: Degradation of threaded fasteners in the reactor coolant pressure boundary of PWR plants.
A detailed review of maintenance work of bolted closures of the reactor coolant pressure boundary (RCPB) has been made for Sequoyah units 1 and 2 from the date of fuel loading of each unit. The enclosed table provides a listing of bolted closures within the RCPB that have experienced leakage.
From the inspections made, the plant has not identified any bolt corrosion as stress corrosion cracking. Sequoyah has experienced erosion-corrosion of fasteners from leakage of borated water, and these were replaced.
Maintenance procedures to detect and replace those fasteners which have signs of corrosion have been implemented.
Sequoyah utilizes only two types of fastener lubricants in the RCPB. These are Neolube and Never-Seez - Pure Nickel Special, nuclear grade (see enclosed table).
