O APPENDIX A NOTICE OF VIOLATION Sacramento Municipal Utility District Docket No. 50-312 P. O. Box 15830 License No. DPR-54 Sacramento, California 95813 As a result of the inspection conducted on May 10-14, 1982, and in accordance with the NRC Enforcenent Policy,10 CFR, Part 2, Appendix C, 47 FR 9987 (March 9, 1982), the following violation was identified:
10 CFR_50, Appendix B, Criterion V, states, in part, that " Activities affecting quality shall be prescribed by documented instructions, procedures, or drawings, of a type appropriate to the circumstances and shall be accomplished in accordance with these instructions, procedures, or drawings."
Quality Assurance Procedure Number 14, Revision 6, of the SMUD Quality Assurance Manual states that the purpose of the procedure is, "To establish a system of periodic calibration to assure the accuracy of ' instruments, I
gauges and measuring devices used in the operation of Rancho Seco Unit No. 1.
Engineering and Inspection Instruction (EII) No. G-6, Revision 0 states that, "The purpose of this procedure is to assure that tools, gauges, instruments and other measuring and test equipnent used at Rancho Seco during construction and major modifications are properly controlled, calibrated, and adjusted to maintain accuracy within necessary limits." In paragraph 2.1 that, "The scope of this procedure covers those Rancho Seco measuring and test -
equipnent and calibrated tools which are maintained by Generation Engineering and required in support of construction requirements used in activities affecting quality class. A master equipnent list of these calibrated iteas shall be maintained by Generation Engineering Inspection Department."
Contrary to the above on May 12, 1982, the Gilson shaker and screens used
~by the onsite testing laboratory for performing coarse aggregate tests were found to be out of calibration. Calibration for this equip.nent was due on April 29, 1982. On May 12, 1932 coarse aggregate sample no. AGO 20.1 was tested using this equipment. Sample No. AGD-20.1 represented material used for the Nuclear Service Electrical Building's Concrete Placement No. 23.
In addition, contrary to Engineering and Inspection Instruction No. G-6, no master equipment list existed for measuring and testing equipment used in construction activities.
This is a Severity Level V Violation (Supplement II).

Pursuant to the provisions of 10 CFR 2.201, the Sacramento Municipal Utility District is hereby required to submit to this office within thirty days-of the date of this Notice, a written statement or explanation in reply, including:
(1) the corrective steps which have been taken and the results achieved; (2) corrective steps which will be taken to avoid further items of noncompliance; and (3) the date when full co.npliance will be achieved.
Consideration may be given to extending your response ti.ne for good cause shown.
