t APPENDIX A NOTICE OF VIOLATION Baltimore Gas and electric Company Docket No.

50-317 Calvert Cliffs, Unit 1 License No. DPR-53 Based on the results of the inspection conducted on June 16-18, 21, 22, 1982 and in accordance with NRC Enforcement Polciy, 47 FR 9987 (March 9, 1982) the following violations were identified:

A.

10 CFR 50 Appendix B, Criterion V and FSAR Article IC.5, Revision 38, April 30, 1975 require that activities affecting quality be accomplished in accordance with documented instructions and procedures.

Calvert Cliffs Instruction (CCI) 200 F, " Maintenance Requests" requires the use of a maintenance request form to authorize and document the performance of work on safety related equipment.

Contrary to the above, an activity affecting quality was apparently accomplished sometime between April 15, 1982 and June 18, 1982 without management knowledge or administrative controls: Two of four safety related containment pressure sensing lines were found ;solated with pipe caps without procedural controls to assure removal and restoration of system operability.

This is a Severity Level IV violation (Supplement I) applicable to DPR-53.

Contrary to the above, procedure STP 0.55-1, Revision 15, Containment l

Integrity Verification did not include a verification of penetration 42 which includes a drain valve and cap as a containment system boundary. In addition, 01-24, Revision 11, Spent Fuel Pool Cooling System only specified that the subject drain valve (SFP-3) be shut and did not require the installed cap as a second containment system leakage limiting barrier.

This proved to be inadequate maintenance of containment system integrity due to significant leakage discovered during the Containment Overall Integrated Leakage Rate Test on June 19, 1982.

This is a Severity Level IV violation (Supplement I) applicable to DPR-53.

Pursuant to the provisions of 10 CFR 2.201, Baltimore Gas and Electric Company is hereby. required to submit to this office within thirty days of the date of this Notice, a written statement or explanation in reply, including: (1) the corrective steps which have Leen taken and the results achieved; (2) corrective steps which will be taken to avoid further violations; and (3) the date when full compliance will be achieved. Where good cause is shown, consideration will be given to extending your response time.
