Identify those bolted: closures of the RCPB that have experienced leakage, particularly those locations.where leakage occurred during the most recent plant operating cycle.
Describe the inspections made and corrective measures taken to eliminate the problem.
If the leakage was attributed to gasket failure or its design, so indicate.

RESPONSE
During routine ISI inspection, 10" accumulator check valves 897-A (Accumulator #31) and 895-B (Accumulator #32) were found to have possible in-dications of leakage.
The Indian Point 3 plant is presently in a refueling outage and the extent of repair has not. yet been ascertained.
If required.
planned corrective action would require removal of the bonnet; visual in-spection of the seal, seal area, and closure studs; and repair or replacement as necessary In addition, as part of plant Preventive Maintenance, the steam generator primary manway closure studs are being replaced.
Although minor leakage has occurred at the primary manways it has not been attributable to closure stud degradation.
No other bolted closures of the RCP3 within the scope of I. E.Bulletin 82-02 have been discovered to have experienced leakage.
ACTION ITEM 3b:
Identify those closures and connections, if any, where fastener lubricants and injection sealant materials have been or are being used and report on plant experience with their application, particularly any instances of SCC of fasteners.
Include types and composition of materials used.
RESPONSE
For lubrication of RCPB closure studs, two products have been and are currently in use at IP3:
1.
Fel-Pro Nuclear Grade N-1000, a high purity coppergraphite anti-seizure lubricant.
2.
Fel-Pro Nuclear Grade N-5000, a high purity nickelgraphite anti-seizure lubricant.
These lubricants do not contain molybdenum disulfide (MoS ) and contain 2
less than 25 ppm sulfur.
IP3 has extensively employed the 'use of ithese lubricants and has experienced no noticeable degrading effects from any of its applications.
Injection sealant materials have not been used on any RCPB closures of the _ type identified in the scope of I.E. Bulletin No. 82-02.
