NOTICE OF VIOLATION i

Northern Indiana Public Service Company Docket No. 50-367 Based on the inspection conducted on October 3-6, 1978, it appears that I

certain of your activities were in noncompliance with the NRC require-ment as noted below. This item is an infraction.

10 CFR 50 Appendix B, Criterion XI; BAILLY PSAR, Appendix D, Paragraph D.2.11; NIPSCo QA Manual,Section XI, and ANSI N45.2 require the licensee and contractor to assure that all prerequisites for the given test have been met and that adequate test instrumenta-tion is available and used.

Thatcher Engineering Corporation (TEC) quality assurance manual, Section 1, states in part. "this manual intends to meet ANSI standard N-45.2."

Paragraph 12 of ANSI N45.2 states in part, " Test Procedures shall include provisions, for assuring that prerequisites for the given test have been met, that adequate instrumentation is available and used, and that necessary monitoring is performed. Prerequisites include such items as calibrated instrumentation, appropriate equipment, trained personnel, condition of test equipment and the item to be tested, suitable environmental conditions, and provisions for data aquisition. Test results shall be documented and evaluated by responsible authority to assure that test requirements have been satisfied."

On October 4, 1978, the inspectors witnessed the vertical load test of indicator Pile AB-155 and found the following to be contrary to the above test control requirements:

No evidence was available to show that the Baldwin-Southwork compression machine, which was used to calibrate test instruments (Load Cell "H" and hydraulic jack #C822418) had been calibrated as required by Thatcher Engineering Corporation (TEC) QA Manual, Section 12, paragraph 2.0.

The inspector contacted the site on October 16 and 17 for supplemental information; however, there was still no evidence that the machine was properly calibrated.

Calibration data furnished for Load Cell H covered the range 0 - 500 tons, however, the test range was 0 - 600 tons. TEC QAP Sections 11 and 12 require proper calibration before testing.

Only one dial gage was used to perform comparison calibration.

TEC QC Manual 12-1 Section 2(B)1 requires that two gages be used.

The hydraulic jack calibration was not performed in the same manner in which it was to be used in the field under actual 1

test conditions. TEC QA Manual Section 12, paragraph 2 requires i

that the calibration methods will be based on the intended project use.

The vertical load test used Load Cell "B" and hydraulic jack gages to monitor the load applied to the pile. At the end of the test the load cell indicated 600 tons and the jack 540 tons even though both were calibrated on the same machine. After the load was removed the load cell read approximately 10 torts, yet, the test data recorded on test report Form VPT-1, indicated that the load cell returned to zero.

No evidence was available to show that TEC personnel performing calibration comparison checks were qualified as required by TEC QA Manual Section 2 paragraph 2 and Section 10.
