Dear Mr. Grier:

On November 3, 1978, in accordance with 10CFR50.55(e), we noti-ficd Mr. A. Toth of the NRC that a deficiency existed in Sailey Controls power cumplies installed in ten Category I applications, orincipally HVAC analog instrumentation control loops.

This letter serves as our 30-day written report for this deficiency.

DESCRIPTION OF THE DEFICIE!TCY Bailey Controls was advised by their pouer sunply vendor (ITT North Electric) that random premature failures of the power cun-j plies could occur due to contanination of a capacitor by the Q

packaging material used by the vendor.

The packaging material V

of concern is foamed-in-place urethane which may da= age the capacitor's dielectric material.

I' A review of the part numbers supplied by Bailey (BCC Part Nos.

8080B02P008 through 3080B02P0012, inclusive) determined that such power supplies were installed in ten Category I applica-i tions.

These applications are prheily IWAC analog instru-mentation control loops such as RBS3V, CRAC, etc.

These syste=s j

are important for post LOCA and off-site release control.

4 CORRECTIVE ACTION 1

l All potentially defective power sunplies will be replaced.

Bailey Control's has notified us that replacement power supplies I ;

will be shipped to us in early December 1973.
