Dear Mr. Keppler:

This letter and its Attachment respond to item 3 of NRC IE Bulletin No. 82-02, which requested information on the leakage of bolted closures in the Reactor Coolant Pressure Boundary. The data covet the period of time starting in January,1981 through July 1982. Pages 1 and 2 of the Attachment list the instances ot' leakages and the corrective actions taken in Unit Nos.1 and 2 of the Cook Plant, respectively.

In reference to Item 3(b) regarding the use of sealant materials, Furmanite sealant was used on two occasions to seal leaks on valves NMO-151 and NM0-152 of Unit No. 1.

These valves are pressurizer PORV block valves, and are 3 inch in size.

Valve NM0-151 had a bonnet leak and valve NMO-152 had a valve stem packing leak. Although these two valves are less than 6.0 inches in size, we have included them in the table for information only. In both cases Furmanite sealant was applied during November 1981 and was removed in January 1982 when the valves were repaired using a new gasket and a new packing. 4 primary coolant during plant operation. During the January 1982 repair, the body / bonnet bolts were not replaced since no indication of erosion or corrosion was identified. The body / bonnet bolts on this valve c:e of Chrome-Moly B7 material.

A visual inspection of the Reactor Coolant Pressure Boundary is made at the beginning of every outage while there is still pressure on the system.

If an indication of leakage is noted, insulation is removed, the source of the leakage is noted, and a job order is written to repair the leak.

During repairs, there are inspection " Hold Points" that call for visual inspections to be made by the Maintenance Supervisor and Chemical Section. The Maintenance Supervisor checks for proper assembly and the Chemical Section assures that no foreign materials have entered the system and that cleanliness has been maintained during the repairs. Their signatures are required before work can continue.

A Containment leak inspection is made prior to criticality.

New gaskets are installed whenever a bonnet or manway cover is removed. After gaskets are replaced and the bonnet or manway repositioned, a specific bolting sequence is followed that applies to that particular closure.

Fel Pro N-1000 and Neolube were the lubricants used on closure studs until we were advised by a Westinghouse bulletin (NSD Data Letter DL-82-01, dated January 22, 1982) that Fel Pro N-1000 no longer be used. The Plant has since banned the use of Fel Pro N-1000 and made Fel Pro N-5000 a new approved lubricant for safety related systems.

A body to bonnet leak was noted on NMO-151 during hot stand-by prior to reactor criticality'.

Furmanite Corp. was called in to seal the leak. The injection sealant was successful in stopping the leak.

Furmanite sealant composition is proprietary information, but is approved for use on nuclear systems.

Typically, during the next outage af ter a valve has been furmanited, the valve is repaired.

The Furmanite sealant is removed and the leak is repaired by installing a new gasket / packing.

All manway bolts were U.T'd and found acceptable. Two closure bolts were steam cut due to the leaking gasket---both bolts were replaced.

The pressurizer manway cover adjacent to the gasket seating surf ace was steam cut and the incications were ground out and Magnetic Particle tested.

The NDE was acceptable.

Two studs were steam cut and replaced.

The gasket was the cause of leakage and only minute traces of boron were found at the manway.
