J APPENDIX A NOTICE OF VIOLATION b

Baltimore Gas and Electire Company Docket Nos. 50-317 50-318 This refers to the inspection conducted by representatives of the Region I (Philadelpnia) office at Units 1 and 2, Calvert Cliffs Nuclear Power Plant, of activities authorized by NRC License Nos. DPR-53 and DPR-69.

These items are infractions.

A. Technical Secification 6.11. " Radiation Protection Program" states,

" Procedures for personnel radiation protection shall be prepared consistent with the requirements of 10 CFR Part 20 and shall be

, approved, maintained and adhered to for all operations involving personnel radiation exposure."

Contrary to this requirement, Procedure RCP-2-402, " Calibration and Maintenance Records (for Health Physics Instrumentation)" was not adhered to on August 10, 1978, in that three instruments (two PIC-6A's, Serial Numbers 1348 and 1342; one teletector, Serial Number

2) were found to be in service beyond the date that the instruments i were due for calibration; the instruments were not marked with a .

rejection sticker or segregated to prevent use in accordance with '

the specifications of the procedure.

B. Technical Specification 6.4, " Training," requires that a retraining and replacement training program for the facility staff shall be maintained under the direction of the Chief Engineer and shall meet ,

or exceed the requirements and recomendations of Section 5.5 of ANSI N18.1-1971. Section 5.5 of ANSI N18.1-1971 requires that means be provided in the training program for appropriate evaluation of its effectiveness.  ;

Contrary to this requirement, Procedure CCI-610A, " Personnel Training Records", which provides the means for the appropriate evaluation of the effectiveness of the on-the-job training program for the qualification of Radiation and Chemistry technicians pursuant to CCI-607B, was not incorporated into the training program defined by CCI-607 Band subsequently was not utilized in the training of Radiation '

and Safety technicians.  !

Appendix A 7.

C. 10 CFR Part 20.103, " Exposure of individuals to concentrations of radioactive materials in air in restricted areas", requires in Section (c) that when respiratory protective equipment is used to limit inhalation of airborne radioactive material, allowance may be made for such use in estimating exposure provided that such equipment is used as stipulated in Regulatory Guide 8.15. Section C.8.a. of Regulatory Guide 8.15 requires that respirable air of approved quality and quihtity is to be provided as specified by NUREG-0041, Section 9.8.

NUREG-0041, Section 9.8 states, " Procedures for maintenance of a supply of r2spirable air are to be included as part of the respiratory protective equipment program; compressed gas cylinder supplies shall be maintained and used in accord with appropriate standards and recomendations."

Contrary to the above requirement, allowance was made for use of supplied air respiratory protective equipment but procedures for the maintenance of a supply of respirable air, in compressed gas cylinders, and for verifying the quality of this air, were not developed or implemented as part of the respiratory protective program.
