Based on the results of the NRC inspection conducted on August 8-11, 1
1978, it appears that certain of your activities were not conducted in w7_:
full compliance with NRC requirements as indicated below. These items have been categorized as described in our correspondence to you dated December 31, 1974.
Criterion V of Appendix B to 10 CFR 50, as implemented by the FSAR; Paragraph 17.1.5, requires in part, " Activities affecting quality shall be prescribed by documented instructions, O.
procedures,... and shall be accomplished in accordance with these instructions, procedures,....".
In addition, Daniel Construction Company work procedure WP-VII-05, Rev. 4, paragraph 3.6-11 restricts the type of tape allowed to be used on stainless steel piping and does not allow use of tape commonly referred to as
" duct" tape.
Contrary to the above requirements, the following items were observed:
On August 9, 1978, during inspection of reactor coolant Loops A and B, the inspector observed that sheets of plastic used for cleanliness protection were attached to the stainless j
steel piping with " duct" tape.
This is an infraction.
