Dear Mr. Seidle:
This letter is written in response to your letter of June 17, 1982, which transmitted Inspecton Report STN 50-482/82-07. As requested, the violation documented in the report is being addressed in three parts:
a)
Corrective steps which will be taken and the results achieved; b)
Corrective steps which will be taken to avoid further violations; and c)
The date when full compliance will be achieved.
Finding:
During review of chemical and volume control system piping hangers by the SRI, Hangers BG01-R007 and GB01-R008 had been observed on l
March 19, 1982, without QC hold tags, while on April 7, 1982, hold tags were seen attached. Upon inquiry, it was learned that both hangers were, and continue to be, on QC hold for inadequate welding to embedments. Deficiency Reports (DRs) 1SD8484MW and ISD8485MW outline the discrepancies and required repairs. KG&E QA personnel, in a surveillance report (S-483) established that required repairs had been completed for Hanger BG01-R007 only on March 30, 1982; however, the DR had neither been reinspected nor closed out when further welding work of attachments outside the scope of both DRs was performed on April 1,1982, on both hangers.

Mr. W.C. Seidle KMLNRC 82-218 July 1, 1982 Daniel Procedure AP-Vl-02, Revision 13, paragraph 3.6 requires issuance of a " Conditional Release" to allow additional work to be performed on parts or components under QC hold outside the identified discrepancy. No conditional release was issued for this additional work on both hangers outside the scope of the DRs.
Response
a)
Corrective steps which will be taken and the results achieved:
A Daniel International Corporation (DIC) Quality and Construction Meeting was conducted on May 18, 1982 to l
discuss and reaffirm the Project position with respect to the application of Quality Holds. The DIC Project Quality Manager instructed Quality Inspection Supervision on May 19, 1982 to be definitive in identifying the exact deficiency on Quality Hold Tags placed beginning May 20, 1982. The extent of the Quality Hold will be applicable to the deficiency identified specifically on the Hold Tag.
Nonconformance Reports 1SN4803PW and ISN4804PW have been issued to document craft personnel working through Quality Holds established by Deficiency Reports ISD8485 and ISD8484.
b)
Corrective steps which will be taken to avoid further violations:
As stated in a) above, Quality Inspection will be definitive in identifying the exact deficiency, and thus identifying the extent of the Quality Hold on all Hold Tags placed beginning May 20, 1982. To prevent further violations of Quality Holds, a training film will be developed and will define the correct application of Quality Hold Tags, the Quality Inspector's responsibility in defining the deficiency in detail on Hold Tag and craft personnel's responsibility in identifying the item (s) on Quality Hold by their observation of Hold Tag limitations.
All Quality Inspection personnel and all craf t personnel through the positions of Certified level I and foremen respectively will be required to attend a training class where the film will be presented, attendance documented, and the Quality Hold philosophy explained in accordance with construction procedure AP-VI-08, Identification and Status of Materials, Parts, and Components. Additionally, i
AP-VI-08 will be revised to include more detailed instruction to the inspector concerning Hold Tag generation.
