Appendix A NOTICE OF VIOLATION Based on the results of NRC inspection on August 16-18, 1978, it appears that certain of your activities were not conducted in full compliance with conditions of your NRC Facility License No.
R-87 as indicated below. The item is categorized as a deficiency.
Paragraph 4 of the Facility License states, that the license is deemed to contain Section 50.54 of Part 50 of 10 CFR which requires anyone that operates the reactor to have a valid operating license at that facility.
Contrary to the above, the reactor control rods were individually moved July 28, 1978 to perform a surveillance test by an operator whose license had expired on July 22, 1978.
