NOTICE OF VIOLATION Based on the results of the NRC inspection conducted August 7-11, 1978, it appears that certain of your activities were not conducted in full compliance with conditions of your Operating License No. DPR-34 as indicated below:

Criterion V of Appendix B to 10 CFR 50 requires that activities affecting quality shall be prescribed by documented procedures and shall be accomplished in accordance with these procedures.

The licensee's approved QA plan, Final Safety Analysis, Section B.5(c), states in part, "The various aspects of plant operation will be defined and controlled by written procedures...."

a.

Administrative Procedure ADM-30, Revision 2, dated September 24, 1976, requires that if items must be unpacked at the work area, the combustibles shall be removed to suitable storage imediately.

Contrary to the above, the Auxiliary Electric Room was observed to contain several cardboard boxes, empty or containing only styrofoam packing.

Administative Procedure ADM-28, Revision 9, dated June 26, 1978, requires that recorders be checked once each shift to be sure that they are inking and reading properly and that a Plant Trouble Report (PTR) be written if the recorder is not working properly.

Contrary to the above, recorders TR 4637 and TR 4638 were not functioning properly in that one or two of the three pens failed to mark during the periods of March 11-17, 1978 and March 10-17, 1978, respectively, and no Plant Trouble Reports were prepared.

This is an infraction.
