obtained, including the discrepancies and the personnel intervievs' conducted,.

Toledo Edison.has concluded that some firevatch log entries were falsified by two individuals..As a result, consistent with company policy,' the employees

. involved vere discharged.: Based on the security investigation results and the-subsequent personnel interviews, Toledo Edison has confidence _that this ' concern vas limited =to the two employees and not indicative of a generic problem in the firevatch program.

The subject firevatches vere established on July 24, 1990 as a conservative entry into the action statements of Technical Specification 3.7.10.

Subsequently, Toledo Edison developed and submitted and the NRC concurred with an interpretation _(Serial Number 1841) concluding that the continuous fire vatches implemented were in excess of actual Technical Specification requirements. Therefore, the firevatches missed as a result of this incidence are not reportable as a Licensee Event Report. However, due.to the potential significance of employee misconduct, Toledo Edison decided a voluntary report should be submitted.

3 Toledo Edison believes that the failure to perform roving continuous firevatches vithin the 15 minute time. period required by.the firevatch procedure was due to

' misconduct by two employees.

Toledo Edison's investigation provided reasonable o

assuranceLthat there were no generic problems with the firevatch program..

Missed ~firevatches by contract employees were previously reported by Toledo.

Edison'in LER 87-005, however, the cause was attributed to firevatch program deficiencies.

Corrective actions implemented for these deficiencies vould not be expected to' preclude the isolated incidences of unauthorized employee conduct described above. 'It'is believed'that the corrective action taken for the.

-i current incident.(i.e., discharge of employees) vill be effective in preventing.
