In accordance with Appendix A, Beever Valley Unit #2 Technical Specification 3/4.4.5, " Steam Generators," Surveillance Requirement 4.4.5.5.a, the following Special Report is submitted.

This report documents the-results of the Inservice Inspection of the steam generator tubes, performed during the Second Refueling Outage.

On September 30,

1990, the inservice inspection of twenty percent of the tubes' in steam generators 2RCS-SG21B (S/G B) and 2RCS-SG21C (S/G C) was completed.

The inspection results identified no indications of tube wall degradation of forty (40) percent or greater.

Two (2) tubes were plugged (leak limiting plugs in the cold leg) as n result of an analysis conducted to determine which tubes could be susceptible to

, vibration induced fatigue.

The tubes were located i'a Row 9, Column 84 in each steam generator (S/G B and C).

  • Page 2 Additionally, plug retainers were installed in nine Westinghouse ribbed plugs on the outlet side (plug retainers had been installed on the inlet plugs during the first refueling).

Three inlet side Babcock & Wilcox rolled plugs were replaced with plugs of Inconel 690.
