Advanced Medical Systems, Inc., wishes to renew our Certificate of Compliance No. 9049, which has an expiration date of December 31,-1990..
Enclosed please find six copies of a consolidated renewal application in our name.. There have been no changes in design of the package since our receipt of Revision 6 of our Compliance Certificate, a copy of wh'.ch is attached for your convenience. Advanced Medical Systems purchased the package from GE in 1983 and has been using it since that time.
