Gentlemen:

SUBJECT: NOTICE OF VIOLATION ,

(NRC INSPECTION REPORT NOS. 50-327/93-42 AND 50-328/93-42)

This refers to the routine resident inspection conducted by W. Holland of this

office from September 4 through October 10, 1993. The inspection included a

review of activities authorized for your Sequoyah facility. At the co.clusion

of the inspection, the findings were discussed with those members of your

staff identified in the enclosed inspection report.  !

Areas examined during the inspection are identified in the report. Within

these areas, the inspection consisted of selective examinations of pro u dures  :

and representative records, interviews with personnel, and observation of

activities in progress. '

Based on the results of the inspection, certain of your activities appeared to I

violate NRC requirements as described in the enclosed Notice of Violation. We-

are concerned about the process that you use to control work' activities around-

and on energized electrical equipment. Past electrical event evaluations j

should have resulted in corrective actions' prior to the event discussed in

this report. In addition, we are concerned about your staff not following i

administrative requirements for post-maintenance or modification testing as l

required.

We are also concerned about a weakness which the NRC identified regarding the .!

lack of a periodic inspection of. critical portions of the inner containment'  !

liner. As of the end of the inspection period, we understand _that future  !

periodic inspections of the 'affected areas will be incorporated during

subsequent refueling outages,

of this letter and its enclosures will be placed in the NRC's Public Document .
