c On November 8,1993, in a telephone discussion, Wayne Hodges, Director, Division of Reactor Safety, informed Thomas P. Noonan, General Manager, Nuclear Engineering and Safety, of our,
decision to dispatch an Augmented Inspection Team (AIT) to review the circumstances and--
evaluate the safety significance of the recent failures of the emergency diesel generator load sequencers at Beaver Valley Power Station Unit 2. Both the No. I and No. 2 emergency diesel generators failed the auto-load surveillance test on. November 4,1993, and November 6,1993, respectively. The test failed when loads did not sequence on the emergency bus during a simulated loss of offsite power with a safety injection signal. The A.T was initiated because of the comm.on-mode failure identified in the emergency diesel generator system, the possiNe generic implications that may exist for the design and operation of the relays used in the sequencer circuit, and the need to fully understand the root cause(s) of these failures. The. AIT, led by James Trapp of our office, is expected to commence their activities at the Beaver Valley Power Station on November 9,1993.
Pursuant to the telephone conference between Mr. Hodges and Mr. Noonan, it is our understanding that you are taking the following actions:
L Quarantine and suspend testing of relays and components, which may have caused the failure of the emergency diesel generator load. sequencers, until resumption is authorized by Mr. Trapp, alt Team Leader.

Maintain Unit 2 in the Cold Shutdown Mode until you receive authorization from the.
Regional Administrator, NRC Region 1.
