SUBJECT: NOTICE OF VIOLATION
(NRC INSPECTION REPORT NO. 50-160/13-02)
This refers to the inspection conducted by C. Rapp and C. Bassett of this
office on September 23-24, and 27-30, 1993. The inspection included a review
of activities authorized for your Georgia Institute of Technology Research
Reactor. At the conclusion of the inspection, the findings were discussed
with those members of your staff identified in the enclosed report.
Areas examined during the inspection are identified in the report. Within
these areas, the inspection consisted of selective examinations of procedures
and representative records, interviews with personnel, and observation of
activities in progress.
During this inspection, an issue was identified relating to your methodology
used to determine Shim Safety Blade reactivity worth. This issue remains:
open; therefore, please submit an analysis, and any other pertinent
information, in a separate correspondence within 60 days of receipt of this
letter.
Based on the results of this inspection, certain of your activities appeared
to be in violation of NRC requirements, as specified in the enclosed Notice of
Violation (Notice). One involved failure to follow procedures for performing
radiation level and contamination level surveys and for completing and
maintaining shipping papers for radioactive material shipments.. - Another dealt
with failure to follow 49 CFR Part 172 requirements for listing the chemical.-
form and failure to include a 24-hour monitored emergency telephone number on
the shipping papers of shipments of radioactive materials. Also, the Nuclear
Safety Committee was not auditing the operator training program as required by
Technical Specification 6.2.e(11).
You are required to respond to this letter and should follow the instructions
specified in the enclosed Notice when preparing your response.
In your
response, you should document the specific actions taken and any additional
actions you plan to take to prevent recurrence. After reviewing your response
to this Notice, including your proposed corrective actions and the rerults of

future inspections, the NRC will determine whether further NRC enforcement
action is necessary to ensure compliance with NRC regulatory requirements.
