Based on the results of this inspection, certain of your activities appeared
to deviate from your commitments in the Safety Analysis Report, as specified
in the enclosed Notice of Deviation (Notice). We are concerned about the
deviations because failure to administratively control nitrogen isolation
valves in accordance with Table 5 of the Safety Analysis Report posed a
potential for compromising containment integrity. Additionally, the potential
to degrade safety-related piping in Level D storage areas existed.
It is our understanding that, during an exit meeting conducted on
October 7, 1993, Mr. J. Vandergrift, Unit 1 Plant Manager, committed to
corrective actions delineated in Section 3.2 and Section 3.4 for unlocked
closed containment isolation valves _ and Level 0 storage of items,
respectively.
