VIOLATION:

l During an NRC inspection conducted on July 29 through September 16,1993, a violation of NRC requirements was identified. In accordance with the " General Statement of Policy and l

Procedure for NRC Enforcement Actions," 10 CFR Part 2, Appendix C, the violation is listed j

s Contrary to the above, maintenance procedures used to repair the 0 diesel generator cooling water pump oil fill line on March 5,1985, (work request 47009) and the 1A diesel generator

]

l cooling water pump oil fill line on February 3,1986, (work request 55783) did not include l

critical tolerances necessary to adequately reset oil levels that supplied the pump bearings.

This is a Severity Level IV violation (Supplement I).

REASON FOR VIOLATION:

The vendor information referenced in Inspection Report 373(374)/93022, (a letter dated March 8,1984 from T. A. Cingcade, Crane Deming Pumps, to D. C. Schafer, CECO),

was incorporated into work packages after its receipt. This information stated that for model 5064 pumps, the oil level should be set 2-1/8 inches below the centerline of the shaft. The information stated that for model 5063 pumps, the level should be set 1-27/32 inches below the centerline of the shaft. The drawing that was attached to the letter showed (through pictorial reference) that the desired oil level should be 1/16 inch above the insih oil ring face as it hangs on the shaft. In 1984, we did not j

recognize this 1/16 inch referencing of the desired oil level to the inside oil ring face as a " tolerance" given by the manufacturer, but rather a specification of comparison.

The instructions for the work requests (L47009 and L55783) mentioned in the violation, included the 1-27/32 inch oil level setting (the distance from the centerline of the pump shaft to the top of the oil specified in the letter) for the oiler level since they involved the model 5063 pumps. The 1/16 inch specification above the oil ring inner face was not stated since the required distance from the centerline of the shaft was specified in the instructions by referencing a drawing in the work package. The work requests were completed with appropriate instructions for oiler setting.

The root cause for not including the " tolerance" in the identified work requests resulted from the opinion of the work package writer that it was redundant information and it was not considered a " tolerance" LaSalle 1

CORRECTIVE ACTIONS TAKEN AND RESULTS ACHIEVED Based on the Oiler setting problems at Quad Cities Station, Lasalle Station System Engineers performed an Oiler inspection of Safety Related Pumps. During the inspection discrepancies were discovered on 0,1A,2A Diesel Generator Cooling Water Pumps. The bearings were inspected on the following work requests:

L23707 (completed 7/30/93): 2DG-01P Outboard bearing was inspected and grease residue was left from when pumps were converted from grease bearings to oil. It was then decided to perform an inspection on IA Diesel Generator Cooling Water Pump (IDG-01P).

i L23738 (completed 8/02/93): 1DG-01P Inboard Bearing cage broken and laying in the bearing housing. The bearing was replaced. The decision was made to inspect the Unit O Diesel Generator Cooling Water Pump (0DG-01P).

L23740 (completed 8/04/93): ODG-01P, no problems were i'ound. As a preventative measure the pump was completely rebuilt.

All 3 work packages contained post maintenance verifications to verify proper oiler setting.

CORRECTIVE ACTIONS TO BE TAKEN TO AVOID FURTHER VIOLATIONS Mechanical Maintenance revised procedure LMP-GM-50, " Crane Deming Model 5063/5064 Pump Maintenance" Revision 3, October 4,1993, to reflect an oiler setting tolerance of t 1/16 inch for both, model 5063 and model 5064, pumps. Contact was made with the vendor on September 20,1993 to verify that the tolerance was still correct, and documented in a telephone memorandum.

1 DATE WHEN FULL COMPLIANCE WILL BE ACHIEVED Full compliance was achieved en October 4,1993 when procedure LMP-GM-50, " Crane Deming Model 5063/5064 Pump Maintenance", was revised.
