SUBJECT: COMBINED INSPECTION REPORT NOS. 50-352/93-24 AND 50-353/93-24
This letter refers to the NRC review in response to the September 7,1993, loss of one
offsite source event at the Limerick Generating Station, in Limerick, Pennsylvania. .The
review was conducted by Mr. A. L. Della Greca and T. A. Easlick of this office on
September 8 through 14,1993, at the Limerick site. At the conclusion of the inspection, on
September 14, 1993, Mr. Della Greca held an exit interview with Mr. R._ Boyce and other
memters of your staff. At that time, the scope and the results of the inspection were
discussed.
The inspection was directed toward areas important to public health and safety. The areas
examined are described in the NRC Region I Combined Inspection Report that is enclosed
with this letter. Within this areas, the inspection consisted of selective examinations of
procedures and representative records, interviews with personnel, and observations by the
inspector.
Our inspection concluded that the loss of one offsite electrical power source and the
subsequent reactor scram were the result of your inadequate control of maintenance activities-
not directly related to the operation of the plant. Poor communications between various -
working groups and between these groups and management rendered the existing controls-
ineffective. Another contributor to the event was your decision to place responsibilities on

the facilities group that were apparently beyond their capabilities and training. Your staff's
activities to identify the causes of the event and to correct the hardware deficiencies
associated with the event were adequate. Also, your immediate and interim corrective
actions put in place to address the above findings, while a full investigation of the event was
ongoing, were considered appropriate.

Following the inspection, on October 4,1993, a public management meeting was conducted
with Mr. D. R. Helwig and other members of your staff to discuss the event, its root causes,
and corrective actions thereof. A ' copy of the slides used by your staff at the meeting is
enclosed as Attachment A. Attachment B is a list of NRC and PECo personnel present at the
meeting.
At the management meeting, your staff concurred that the control of excavation activities by
the PECo facilities was less than adequate and identified the causes leading to the event and
the actions that were or will be taken to address the issues involved.
As a result of the inspection and of the management meeting, two unresolved items,
pertaining to your resolution of the above concerns and to the root cause of the breaker
failure, were identified. They will be reviewed during a future inspection of your licensed
program.
