SUBJECT:
NRC INSPECTION REPORT 50-498/93-34; 50-499/93-34
This refers to the inspection conducted by Mr. R. E. Lantz of this office, and
the personnel identified in the enclosed report, on September 24 through
October 1, 1993. The inspection included an assessment of the qualifications
of 15 individuals who had applied for licenses to operate your South Texas
Project Electric Generating Station, Units 1 and 2. . At the conclusion of the
inspection, findings of generic significance were discussed with you and those
members of your staff identified in the enclosed report. Appropriate licenses
have been issued to 10 applicants who successfully. completed all portions of
their examinations and two licenses are being hel-J pending completion of
experience requirements for license and the generic fundamentals examination
section, as applicable to the individual applicants.
Areas examined during the inspection are identified in the report. Within
these areas, the inspection consisted of evaluation of license applicants by
the use of written examinations, dynamic simulations, selected job related
tasks, and oral questioning in accordance with NUREG-1021, " Operator Licensing
Examiner Standards," Revision 7.
The results of this inspection are
documented on page 2, in the enclosed report.
The NRC is concerned with the apparent inability of some applicants to respond
to shutdown events and procedures. Of the three crews that were evaluated in
a shutdown loss of coolant accident scenario, one was slow to initiate
appropriate corrective actions and one never entered the correct procedure and
never initiated an effective mitigation strategy. This concern is heightened
by the current long term shutdown of both units at your facility and by the
many NRC and industry information sources about the importance of shutdown
events. We understand that this performance weakness will'be incorporated
into your needs determination for future licensed operator training.
In accordance with 10 CFR 2.790 of the NRC's " Rules of Practice,".' a copy of-
this letter and its enclosures will be placed in the NRC Public Document Room.
