SUBJECT: NRC INSPECTION REPORT 50-458/93-28 (NOTICE OF VIOLATION)
This refers to the inspection conducted by W. F. Smith, and the inspectors
identified in the attached report, of this office on November 7 through
i
December 18, 1993.
The inspection included a review of activities authorized
for your River Bend Station facility. At the conclusion of the inspection,
the findings were discussed with those members of your staff identified in the
enclosed report.
Areas examined during the inspection are identified in the report. Within
these areas, the inspection consisted of selective examinations of procedures
and representative records, interviews with personnel, and observation of
activities in progress. The purpose of the inspection was to determine
whether activities authorized by the license were conducted safely and in
'
accordance with NRC requirements.
Based on the results of this inspection, certain licensed activities appeared
to be in violation of NRC requirements, as specified in the enclosed Notice of
Violation (Notice). The first violation is of concern because operator errors
resulted in operating the plant in conditions prohibited by the Technical
Specifications. The second violation is of concern because work instructions
were not followed.
This violation demonstrated that some plant personnel
still do not properly attend to details in accordance with management's
expectations. The third violation is of concern because procedure
inadequacies appeared in work instructions currently being generated.
One unresolved item was identified concerning the use of Generic Letter 91-18-
guidance to substitute manual detection and manual actuation in lieu of
automatic actions that were intended to mitigate the consequences of an

accident.
This matter is discussed in Section 2.3.2 of the enclosed
inspection report.
You are required to respond to this letter and should follow the instructions
specified in the enclosed Notice when preparing your response.
In your
response, you should document the specific actions taken and any additional
actions you plan to prevent recurrence.

Notice, including your proposed corrective actions and the results of future
