l METeQ TESTING LASORATORIES, OF UTAH, INC. 1 388 West Gregson Avenue Sett Leke City, Utah 8411 S (801)487-0801 I i 1.0 ORGANIZATION i 1.1 The final responsibility for the Quality Assurance Pro-l gram for Part 71 requirements rests with Met-Chem Test-ing Laboratories, of Utah, Inc. 1.2 Design and fabrication shall not be conducted under this Quality Assurance Program. The Radiation Safety Officer is designated the responsible individual for the Part 71 l quality assurance requirements. i 1.3 The Radiation Safety Officer is responsible for overall administration of the program, training and certifica-l tion, document control and auditing. 1.4 The radiographers are responsible for handling, storing, shipping, inspection, test and operating status and record keeping. 2.0 QUALITY ASSURANCE PROGRAM 2.1 The management of Met-Chem Testing Laboratories, of l Utah, Inc. establishes and implements this Quality Assurance Program. Training, prior to engagement, for all quality assurance functions is required according to written procedures. Quality Assurance Program revisions will be made according to written procedures with man-agement approval. The Quality Assurance Program will ensure that all defined Quality Assurance Procedures, i engineering procedures and specific provisions of the package design approval are satisfied. The Quality Assurance Program will emphasize control of the charac-teristics of the package which are critical to safety. 2.2 The Radiation Safety Officer shall assure that all radioactive material shipping packages are designed and manufactured under a Quality Assurance Program approved by the Nuclear Regulatory Commission for all packages designed or manufactured after January 1, 1979. l l l 3.0 DOCUMENT CONTROL 3.1 All documents related to a specific shipping package will be controlled through the use of written proced-ures. All document changes will be performed accord-l ] ing to written procedures approved by management. 1 3.2 The Radiation Safety Officer shall ensure that all qual-l ity assurance functions are conducted in accordance with the latest applicable changes to these documents. i 4.0 HANDLING, STORAGE AND SHIPPING l l 4.1 Writter, safety procedures concerning the handling, stor-l age and shipping of packages for certain special form ] radioactive material will be followed. Shipments will i not be made unless all tests, certifications, accept-ances and final inspections have been completed. Work instructions will be provided for handling, storage and shipping operations. 4.2 Radiography personnel shall perform the critical handl-ing, storage and shipping operations. 5.0 INSPECTION, TEST AND OPERATING STATUS l 5.1 Inspection, test and operating status of packages for l certain special form radioactive material will be in-l dicated and controlled by written procedures. Status j will be indicated by tag, label, marking or log entry. j Status of non-conforming parts or packages will be l positively maintained by written procedures. 1 J 5.2 Radiography personnel shall perform the regulatory re-j quired inspections and test in accordance with written j procedures. De-i scriptions of equipment and written procedures will also be maintained. i 6.1.1 The records will be maintained in accordance with a written procedure. 6.1.2 The records will be identified and retrievable. 6.1.3 The Radiation E. ety Officer will maintain a list of the records and their storage location. 1 7.0 Al'DITS l 1 7.1 Established schedule of audits of the Quality Assurance Program will be performed using written check lists. Results of audits wil] be maintained. Audit reports will be evaluated and deficient areas corrected. The audits ) will be dependent of the safety sirnificance of the activity being audited, but each activity will be audit-ed at least once a year. Audit reports will be main-l tained as part of the quality assurance records.
