Dear Mr. Broughton:

SUBJECT: NRC INSPECTION REPORT NO. 50-289/93-25

This letter transmits the findings of the resident safety inspection conducted by Ms. M.

Evans and Mr. D. Beaulieu from October 26,1993 - December 6,1993, at the Three Mile

Island Nuclear Station Unit 1. The inspectors based these findings on observations of

activities, interviews, and document reviews and discussed them with Mr. T. G. Broughton

and other members of your staff. Inspector observations indicated safe and conservative

operation.

Prior to the startup from the 10R refueling outage, one of the pressurizer code safety valves

began to leak. We found that your staff planned to slightly open the valve at power without

a sound technical basis for concluding that the valve would not fully lift. In addition, your

staff did not give sufficient consideration to the relative risks of performing the evolution at

power versus hot shutdown. Although the valve vendor recommended 6is approach and you

believed there was no regulatory prohibition .o performing the activity at power, we raised

,

the issue with senior management, which caused your staff to conde.:t a rigorous review and

independent assessment that resulted in a different and safer approach to the activity. We

understand that your staff is preparing a Plant Experience Report to evaluate this incident.

In accordance with 10 CFR 2.790 of the NRC's " Rules of Practice," a copy of this letter and

its enclosure will be placed in the NRC Public Document Room. We appreciate your
