Dennis Huhgmn Director of Envimamental Health and Safety 825 East Wisconsin Avenue P. O. Box 359 Appleton, Wisconsin 54912
Dear Mr. Hultgren:
Subject:
Special Inspection No.93-014 On September 30,1993, S. Courtemanche and S. Shaffer of this office conducted a special safety inspection at your Camp Hill, Pennsylvania facility of activities authorized by an NRC general license. The inspection involved an examination of your licensed activities as they relate to radiation safety and to compliance with the Commission's mgulations and the conditions of the general license. The inspection consisted of observations by the inspector, interviews with personnel, and a selective examination of mpresentative records. The findings of the inspection were discussed with Mr. Fred M. Gillespie, Jr. and other members of the staff at the Camp Hill, Pennsylvania facility along'with Mr. Dwayne Jones of your staff at the conclusion of the inspection.
l The inspection resulted in the identification of three apparent violations. A letter containing a Notice of Violation was issued to the management of your Camp Hill, Pennsylvania facility
)
addressing the issues at that facility. One of the issues, however, involved management's I
approval of an unauthorized activity after concems were brought to management's attention by
)
workers. Specifically, on May 19, 1993, workers wem directed to mpair/ replace a mylar j
window on an Lippke System Model MV-PM gauge. After mmoving the source housing, the workers notified management that there was a label affixed to the housing that stated that only H
licensed personnel could remove the source shielding or soume containment housing.
i Management apparently considered the information pmvided by the workers and instructed them to mmove the housing despite the prohibition on the label. We are concerned that management did not stop the work after the issue was brought to their attention.

i Appleton Papers, Inc. While we am not aware that similar activities in violation of regulatory requirements have taken place at other Appleton Papers, Inc locations where radioactive gauges are used, we are concerned that management at those locations may not be sufficiently knowledgeable of regulatory and license requirements to ensure that the requirements are being adhemd to.
Therefore, we request that you provide us within 30 days of the date of this letter, a description of the actions that have been taken or will be taken to verify and ensure that management is knowledger.ble of regulatory and license requirements and that those facilities are in compliance with the requirements.
Your cooperation with us is appreciated.
