This inspection included a review of authorized activities for your

Quad Cities Nuclear Power Station facility. At the conclusion of the
inspection, the findings were discussed with those members of your staff
identified in the enclosed report.
Areas examined during the inspection are identified in.the report. Within
-
these areas, the _ inspection consisted of a selective . examination of procedures..

and representative' records, interviews with personnel, and observationiof
activities in progress.
During this inspection, certain of your activities appeared to'be' inLviolation .
of NRC. requirements, as specified in the enclosed . Notice of Violation

(Notice).

You are required to respond to this letter and should' follow the instructions
specified in the enclosed Notice when preparing your response.
Inlyour-
response, you should document the specific actions taken and any additional

actions you plan to prevent recurrence. After reviewing your. response to this

Notice, including your proposed corrective actions-and the results of future-
inspections, the NRC will determine whether further NRC. enforcement action.is
necessary to ensure compliance with NRC regulatory requirements.
During this report period, instances of ' poor procedural adherence, a lack of.
system knowledge, and' poor quality pre-job briefings were identified.
Similar
problems have been noted in other recent NRC inspection reports.
These items
show a need for management action to ensure personnel' are knowledgeable ~of
i
assigned tasks.
As part of your response to the enclosed' Notice, we ask that
I
you specifically address your planned corrective actions regarding management.
