Combined Inspection Nos: 50-213/93-23, 50-245/93-30, 50-336/93-25,

and 50-423/93-27 (11/15-18/93)

This letter refers to the above subject unannounced Fitness-for-Duty (FFD) inspection of the

Haddam Neck Nuclear Power Plant and Millstone Nuclear Power Station Units 1,2, and 3

by Mr. E. B. King of this office and Mr. W. J. Raymond, Senior Resident Inspector,

Haddam Neck Nuclear Power Plant. Areas examined during the inspection are described in

the enclosed inspection report. Within these areas, the inspection consisted of interviews

with personnel, observations by the inspectors, and selective examinations of procedures and

representative records. The inspection was continued in the Region I office on December 9,

1993, to review the licensee's committed response to a programmatic weakness addressed in

a licensee's memo dated December 2,1993.

This inspection found that your FFD program was generally responsive to the spirit and

intent of the FFD rule and that the program was generally aggressive and comprehensive.

Management support for the program was evident in several program enhancements made to

increase effectiveness. However, it appeared that two of your activities were not conducted

in full compliance with NRC requirements as set forth in the Notice of Violation enclosed

herewith as Appendix A. The violations involved (1) failure to establish and implement

written procedures designed to meet the general performance and specific requirements of 10 CFR 26, and (2) failure to properly investigate and report unsatisfactory laboratory

performance testing results. These violations have been categorized by severity level in

accordance with the " General Statement of Policy and Procedure for NRC Enforcement

Actions," 10 CFR Part 2, Appendix C (Enforcement Policy). You are required to respond to

this letter and, in preparing that response, should follow the instructions in Appendix A.
