SUBJECT: NRC INSPECTION REPORT NO. 50-213/93-21
,
This letter transmits the report of the resident safety inspection conducted by Messrs. W.
Raymond and P. liabighorst of this office during the period November 7,1993, to
December 11,1993, at the Haddam Neck Plant, liaddam Neck, Connecticut. The report
z
covers activities authorized by NRC License No. DPR-61. This inspection focused on issues
important to public health and safety. Preliminary findings were discussed with Mr. J. Stetz
of your staff at the conclusion of the inspection.
During this report period, we found Haddam Neck was operated in a safe and conservative
manner, as plant personnel responded to several challenges. Licensed operators performed
very well in responding to degraded equipment conditions. In particular, the operators
'
performed very well to stabilize the plant following a failure of the "B" main feedwater
pump. The plant staff also performed very well to assess new industry data regarding the
!
valve factors for motor operated valves used at Haddam Neck, and to expeditiously completc
modifications that assured the main feedwater isolation valves could perform the intended
'
design basis functions.
The enclosed report also describes two instances where plant system valves were found to be
i
positioned different than intended. Although the mispositioned valves did not result in a
i
system inoperability, the events highlight an apparent adverse performance trend by non-
licensed operators in the control of the plant configuration. It is noted that equipment human
factor denciencies contributed to the mispositioned valve events. Additionally, while
,
unrelated to the valve events and apparently isolated, the enclosed report also describes an
,
event in which a nonlicensed operator failed to accurately maintain the logs documenting his
Dre watch duties for the diesel rooms. Two apparent violations regarding these issues are

described in the enclosure to this letter. You are required to respond to this letter, and'

should follow the instructions in the enclosed Notice when preparing your response.
