SUBJECT: NOTICE OF VIOLATION
(NRC EXAMINATION REPORT NO. 50-416/93-301)
This refers to the Nuclear Regulatory Commission (NRC) administered
examinations led by Jonathan H. Bartley during the period September 27 -
October 1, 1993. The examinations were administered to employees of your
company who had applied for licenses to operate the Grand Gulf Nuclear
Station. At the conclusion of the examination, the examinations and
preliminary findings were discussed with those members of your staff
identified in the NRC Examination Report, Enclosure 2.
A Simulator Facility Report is included in this report as Enclosure 3.
A copy
of the written examination questions and answer key, as noted in Enclosure 4,
was provided to members of your training staff at the conclusion of the
examination.

In February 1992, six candidates from your facility participated in the
Generic Fundamentals Examination. Of the six examined, five passed for an 83
percent pass rate.
In June 1993, five candidates from your facility
participated in thi Generic Fundamentals Examination. Of the five examined,
five passed for a ?.00 percent pass rate.
Based on the results of this inspection, certain of your activities appeared
to be in violatior, of NRC requirements, as specified in the enclosed Notice of
Violation (Notice).
In addition, the enclosed Examination Report identifies
activities that violated NRC requirements that will not be subject to

enforcement action because the licensee's efforts in identifying and/or
-
correcting the violation meet the criteria specified in Section VII.B of the
You are required to respond to this letter and should follow the instructions
specified in the enclosed Notice when preparing your response.
In your-
response, you should document the specific actions taken and any additional
actions you plan to prevent recurrence. After reviewing your response to this
Notice, including your proposed corrective actions and the results of future

inspections, the NRC will determine whether further NRC enforcement action is
necessary to ensure compliance with NRC regulatory requirements.
