This letter responds to the violation 93-13-05 as cited in the subject NRC inspection report. The violation therein states:
10 CFR 50, Appendix B, Criterion XVI, " Corrective Actions," requires, in part, that measures shall be established to assure that conditions adverse to quality, such as failures, malfunctions, deficiencies, deviations, defective material and equipment and nonconformances are promptly identified and corrected. In the case of significant conditions adverse to quality, the measures estahlished shall assure that the cause of the condition is determined and corrective action taken to preclude repetition. The identification of the significant condition adverse to quality, the cause of the condition, and the corrective action taken shall be documented and reported to appropriate levels of management.
Contrary to the above, as of September 3,1993, measures had not been established assuring that the cause of conditions adverse to quality was determined and corrective action taken to preclude repetition in that the following instrumentation:

ATTACIIMENT Admission or Denial Violation 93-13-05 occurred as described in NRC Inspection Report 93-13.
Reason for Violation The reason for this violation was inadequate procedures in that there was no direction given for actions to be taken when the subject instruments were found in an out-of-tolerance condition.
Corrective Action Taken and Results Achieved All containment cooler inlet and outlet flow instrumentation (sixteen instruments total for both units) have been recalibrated.
Corrective Steps to Avoid Further Violations Procedural guidance will be enhanced or established as appropriate to identify and evaluate repetitive out-of-tolerance conditions on those components used to satisfy TS surveillance requirements and safety related components in the PM program. This procedural guidance will also provide criteria for determining the appropriate corrective action when these conditions are identified.
The containment cooler inlet and outlet flow instrumentation will be evaluated based on i
this guidance once developed.
