We submit this Special Report concerning an inoperable Wide Range Noble Gas Monitor (WRNGM) on the Unit 1 Main Vent Stack as required by Technical Specification 3.3.3.1, Table 3.3-6, Item 2.b.i.
ACTION TAKEN On November 17,1993, the Unit 1 Wide Range Noble Gas Monitor was removed from service at approximately 1425 when the automatic cight-hour check source on the mid-range channel failed.
During the performance of troubbhooting, the check source assembly was found to be malfunctioning. Repair efforts are continuing.
EFFECT ON OPERATION Unit 1 Technical Specification 3.3.3.1 ACTION Statement (30) was entered when the WRNGM was removed from service. In accordance with ACTION 30 and our radioactive gas release procedure (CP-213), the main vent radiation monitor was being used as the alternate preplanned main vent stack monitor for discharge flow-stream effluents. The WRNGM inoperability did not affect Unit 1 operation.
CAUSE OF INOPERAllllIIY
'Ihe cause of the inoperability was a malfunctioning mid-range check source assembly. An investigation is underway to determire the cause of the malfunction.
PLANS AND SCllEDULES FOR RESTORING TIIE SYSTEM TO OPERAllLE The WRNGM was returned to service on December 14,1993 at 1355.
1
