Their letter and QA Program was not dated; however, a check from B&G Inspection (No. 1132) for the application fee.was enclosed and dated September 20, 1993.

Therefore, the QA Program Approval for Radioactive Material Packages No. 0775, Revision No. O, indicates the date of application as September 20, 1993.
