This is to request termination of our Radioactive Material Packages License No. 0399.
We have ceased Industrial Radiography and do not possess any licensed material requiring use of the Model 6717B package.or any other Type B package.
All records concerning this license are being permanently retained at'our Materials and Tests Unit, 1801 Blue Ridge Road, Raleigh,. North' Carolina.
A copy of our terminated agreament state radioactive material license is enclosed.for your record.-
