SUBJECT: NRC INSPECTION REPORT 50-445/93-33; 50-446/93-33
This refers to the inspection conducted by Mr. D. N. Graves, G. E. Werner, and
K. H. Kennedy of this office on August 22 through October 2, 1993, The
inspection included a review of activities authorized for your Comanche Peak
Steam Electric Station, Units 1 and~2, facility. At the conclusion of' the
~
inspection, the findings were discussed with you and those members'of your
staff identified in the enclosed report.
Areas examined during-the inspection are identified in the report. 'Within
these areas, the inspection consisted of selective examinations of procedures
and representative records, interviews with personnel, and observation of
activities in progress. The results of this inspection are documented on
pages 1-2, in the enclosed report.
Your staff identified that a cap on a feedwater line drain valve was missing,
which. as defined in licensing basis documents, was integral to-the.
containment-boundary.
Several system operating procedures did not reflect the
-
requirements of the Final Safety Analysis Report and the design ' basis ' document.
regarding capping of vents and drains inside containment isolation-valve
boundaries and were in violation of Technical Specification 6.8.1.
This
violation is not being cited because the criteria in paragraph VII.B.2 of
Appendix.C to 10 CFR Part 2 of the NRC's " Rules of Practice,"-were satisfied.
Although potential breeches of containment integrity normally_ constitute
significant violations, the safety significance ofxthis particular occurrence
was-1_ow due to the fact that it occurred on a closed; system. .The_ corrective-
actions ~ initiated by your staff.were prompt, comprehensive and should preclude
recurrence. As discussed'during the September 30, 1993, exit meeting, your.
staff committed to-implement the corrective actions specified in paragraph 2.2;
of the enclosed report. ~ If your understanding of this commitment differs from
ours,-please contact-me immediately.
In accordance with 10 CFR 2.790 of the NRC's " Rules of Practice," a. copy. of-
this letter and its enclosures will be placed in the NRC Public Document Room.
