NRC REGION III AUGMENTED INSPECTION TEAM (AIT) REVIEW 0F THE

SEPTEMBER 14, 1993, LASALLE COUNTY STATION, UNIT 1, LOSS OF STATION

AUXILIARY TRANSFORMER

The enclosed report refers to a special onsite review by a United States

Nuclear Regulatory Commission (NRC) Augmented Inspection Team (AIT) on

September 16 through September 21, 1993, relative to the September 14, 1993,

loss of the station auxiliary transformer, at the LaSalle County Station,

Unit 1, and the failures of equipment that occurred during the event. The AIT

was composed of Messrs. J. D. Wilcox, Jr., M. J. Miller, R. A. Winter,

J. H. Neisler, A. J. Kugler, and R. A. Spence of the Region III office, the

Office of Nuclear Reactor Regulation (NRR) and the Office ft.r Analysis and

Evaluation of Operational Data (AE0D). The report also rtfers to the followup

activities of your staff and to the discussion of our fi.di.;p with

Mr. W. P. Murphy and others of your staff at the conclusion of the inspection.

The enclosed copy of our AIT report identifies areas examined during the

inspection. Within these areas, the inspection consisted of a selective

examination of procedures and representative records, observations, and

interviews with personnel.

The AIT was formed to gather information on the event.

Specifically, the AIT

examined licensee response to the event; root causes of the Unit 1 station

auxiliary transformer trip, the reactor scram, and other equipment failures;

evaluated whether there was an electrical design vulnerability; and evaluated

the licensee's notification process and classification of the event.

The AIT concluded that while the event was a significant operational

occurrence, no radiation was released and it did not pose an immediate impact

on the health and safety of the public. Nonetheless, NRC is concerned that

inadequate maintenance resulted in several equipment failures that occurred

during the event and recovery. We note that inadequate maintenance has been a-

contributing factor to other events at LaSalle and we are concerned that your

corrective actions have not been effective.

In view of these events, we

consider it important for you to carefully assess the scope of your current

maintenance activities and practices.

The initial notification and certain aspects of the communication between the

site and the NRC during the event was also identified as being a weakness.

Strengths identified by the AIT included operating crew and technical support

center (TSC) personnel actions to deal with the event, and support provided

them by other Commonwealth Edison organizations.

The AIT charter does not include examining whether NRC rules and regulations

were violated or recommending enforcement action; however, the results of this

inspection could be the subject of a subsequent inspection.

In accordance with 10 CFR 2.790 of the Commission's regulations, a copy of

this letter, the enclosed inspection report, and its enclosures will be placed

in the NRC Public Document Room.
