SUBJECT:

NRC INSPECTION REPORT 50-498/93-30; 50-499/93-30 (NOTICE OF

VIOLATION)

This refers to the inspection conducted by Mr. D. P. Loveless, and the

inspectors identified in the attached report, of this office on August 15

through September 25, 1993. The inspection included a review of activities

authorized for your South Texas Project, Units 1 and 2, facility.

At the

conclusion of the inspection, the findings were discussed with those members

of your staff identified in the enclosed report.

Areas examined during the inspection are identified in the report. Within

these areas, the inspection consisted of selective examinations of. procedures

and representative records, inspection of installed equipment and system

alignments, interviews with personnel, and observations of activities in

progress.

The purpose of the inspection was to determine whether activities

authorized by the license were conducted safely and in accordance with NRC

requirements.

Based on the results of this inspection, certain activities appeared to be in

violation of NRC requirements, as specified in the enclosed Notice of

Violation (Notice). The violation is of concern because safety-related

replacement parts were being issued to plant workers for installation without

proper engineering evaluation and modification.

Your response should include

a discussion of any necessary revisions to the modification process, and the

procedural controls that govern this process.

Additionally, two examples of failure to follow plant procedures were

identified.

The first example involved a breach of a heating, ventilation,

and air-conditioning boundary door without the issuance of a permit.

The

second example involved the failure of plant craftsmen to accept a clearance

order prior to working on a ventilation fan.

These violations are not being

cited because the criteria in paragraph VII.B.1 of Appendix C to 10 CFR Part 2

of the NRC's " Rules of Practice," were satisfied.

These violations were

isolated occurrences and the licensee staff took prompt and effective actions

to correct the problem.
