SUBJECT: NRC INSPECTION REPORT 50-482/93-29 (NOTICE OF VIOLATION)

This refers to the inspection conducted by Messrs. G. A. Pick and

J. F. Ringwald of this office from October 24 to December 4, 1993. The

inspection included a review of activities authorized for your Wolf Creek

Generating Station facility. At the conclusion of the inspection, the

findings were discussed with those members of your staff identified in the

enclosed report.

Areas examined during the inspection are identified in the report. Within

these areas, the inspection consisted of selective examinations of procedures

and representative records, interviews with personnel, and observation of

activities in progress. The purpose of the inspection was to determine

,'

whether activities authorized by the license were conducted safely and in

accordance with NRC requirements.

Based on the results of this inspection, certain licensed activities appeared

to be in violation of NRC requirements, as specified in the enclosed Notice of

Violation (Notice).

Violation A pertains to four instances of failure of

licensed operators to maintain cognizance of control panels and permits. Your

staff immediately initiated appropriate corrective actions when the events

occurred.

Because of your recent initiatives to reduce operator errors and

ongoing corrective actions, as discussed at a public meeting on November 9,

1993, no response tc this violation is required.

Violation B pertains to two examples of failure to properly restore from

clearance orders by maintenance personnel that resulted from a breakdown in

coordination and communication. This violation is of concern because improper

equipment clearances are hazardous to both plant and personnel safety.

Violation C resulted from identification by NRC of inadequate corrective

l

actions implemented within the maintenance department related to clearance

!

orders. This is of concern because personnel failed to recognize that the

corrective actions applied within a single maintenance group affected the

entire department.

l

Although 10 CFR 2.201 provides for a written statement of explanation to this

office within 20 days of your receipt of this Notice, we note that Violation A

f

was corrected and those actions, including your root cause analysis,' were

reviewed during this inspection. Therefore, no response with respect to

",

Violation A is required. You are required to respond to this letter and

should follow the instructions specified in the enclosed Notice when preparing

your response to Violations B and C.

In your response, you should document

t

the specific actions taken and any additional actions you plan to prevent

recurrence. After reviewing your response to this Notice, including your

~

proposed corrective actions and the results of future inspections, the NRC

'

will determine whether further NRC enforcement action is necessary to ensure

compliance with NRC regulatory requirements.

During this period, the inspector identified that instrumentation and control

personnel inappropriately dispositioned repair of a component on a work

.

request that violated licensee procedures and Technical Specification 6.8.1.a.

This violation is not being cited because the criteria in paragraph VII.B.1,

'

Appendix C to 10 CFR Part 2 of the NRC's " Rules of Practice," were satisfied.

This violation had limited significance because of adequate redundant

indicators, and your' staff took appropriate actions in response to the

problem.

It is our understanding that the operations manager committed to upgrade your

slave relay test procedures prior to their next performance but no later than

March 31, 1994.

Personnel will modify the procedures to ensure that operators

can restore components that change position during slave relay tests to their:
