This refers to an Enforcement Conference conducted by Mr.. John B. Martin,

Regional Administrator, and other members of the Region III staff on

December 14, 1993, regarding activities at your Fermi 2 Nuclear Power Station,

!

and to the discussion of our findings with your staff.

This Enforcement

Conference was conducted as a result of the findings of an inspection

conducted from October 12 through November 10, 1993, in which an apparent

violation of NRC regulations was identified.

Inspection Report No. 50-

341/93022, dated December 2,1993, documented the results of that inspection.

The enclosed report summarizes the conference.

As noted during the conference, we are concerned that a longstanding practice

existed that fostered personnel working ahead of established schedules.

This

perceived schedular pressure, which overshadowed the concern for safety, had

.i

'

been identified in your corrective action system as far back as September

1991, without effective corrective action.

Despite senior management

involvement in the plan of the day meeting on September 17, 1993, the

expectation to maintain adherence to the schedule was not effectively

communicated such that the staff felt it was acceptable to release work

packages for work before all plant conditions were met.

In this case, three

maintenance mechanics were burned when they removed the actuator from a

pressurized system when the schedule had called for the system to be

depressurized prior to commencing the work.

You will be notified by separate correspondence of our decision regarding the

Enforcement Action based on the information presented and discussed at the

Enforcement Conference. No response is required until you are notified of the

proposed Enforcement Action.

In accordance with 10 CFR 2.790 of the NRC's " Rules of Practice," a copy of

this letter and the enclosed inspection report will be placed in the NRC

Public Document Room.
