Gentlemen:
SUBJECT: NRC INSPECTION REPORT N0. 50-364/93-400
This refers to the inspection conducted by NRC Mobil Nondestructive Examina-
tion (NDE) Laboratory personnel on October 4-15, 1993. The inspection
included a review of activities authorized for your Farley facility. At the
conclusion of the inspection, the findings were discussed with those members
of your staff identified in the report.
Areas examined during the inspection are identified in the report. Within
,
these areas, the inspection consisted of selective examinations of procedures
i
and representative records, interviews with personnel, and observation of
activities in progress.
The inservice inspection nondestructive examination program, presently
deployed at Farley, was found to be well implemented as confirmed by indepen-
dent testing, discussions with site personnel and ISI-NDE procedure and
program review.
Relative to the discovery of the indication in the feedwater nozzle to pipe
weld (APR2-4350-22), of the feedwater line to steam generator "C," we have
reviewed our radiographic and ultrasonic results, the original archival
radiographs, the radiographs taken by Farley during this inspection and site
.
'
generated ultrasonic data.
In addition, we have applied our own special
techniques and transducers to this indication.
Based on this information, our
experiences with nuclear power stations across the nation, the location and
nature of the weld and the materials of construction; we are characterizing
this indication as a 2" long thermal fatigue crack with approximately 0.3"
throughwall dimension as determined by UT techniques. Subsequent to the
inspection exit meeting, the licensee provided to the Region 11 staff informa-
tion on the operation of the auxiliary feedwater/feedwater system operation
,
that shows the driving force for thermal fatigue crack growth to be relatively
i
low due to the normally low auxiliary feedwater flow rate. Additionally, the
fracture mechanics analysis provided at that time indicates that a crack of
the size noted above, if it were present, would not propagate to unacceptable
-
dimensions prior to the next refuel outage. However, the site NDE staff has
characterized the indications as internal root undercut. Our current opinion

is that the subject area of weld APR2-4350-22 should be examined by appropri-
ate NDE (UT) techniques to track the status of the indication during the next
refuel outage, and sooner if a brief outage provides time for an examination.
,
Within the scope of the inspection, violations or deviations were not identi-
fied.
In accordance with 10 CFR 2.790 of the NRC's " Rules of Practice," a copy of
this letter and its enclosure will be placed in the NRC Public Document Room.
