SUBJECT: DAVIS-BESSE ENFORCEMENT CONFERENCE REPORT NUMBER 50-346/93026(DRP)
This refers to an Enforcement Conference conducted by Mr. John B. Martin and
other members of the Region III staff on December 17, 1993, regarding
,
activities at your Davis-Besse Nuclear Power Station authorized by NRC License
No. NPF-3, and to the discussion of our findings with you and your staff. We
have enclosed a report summarizing the discussion.
You will be notified by separate correspondence of our decision regarding the
Enforcement Action based on the information presented and discussed at the
Enforcement Conference. No response is required until you are notified of the
proposed Enforcement Action.
As discussed at the conference, a concern was identified related to your
apparent misunderstanding of the auxiliary feedwater system design basis. As
a result, on October 20, 1993, with the unit at power, you placed the
auxiliary feedwater system in a configuration such that it could not perform
its safety function for the entire range of postulated accidents as specified
in the Updated Safety Analysis Report. The resulting apparent violation
concerned the fact that both trains of the auxiliary feedwater system were
made inoperath for a period exceeding that allowed by Technical
Specifications.

In addition, as discussed at the conference, concerns were identified related
to your configuration control programs.
Specific examples included:
(1) containment air cooler service water valves were not verified in the
'
correct position as required by plant Technical Specifications, (2) multiple
discrepancies existed between various configuration control documents,
procedures, and personnel understanding of document hierarchy, and (3) you
previously identified these items as problems, but failed to prevent their
recurrence. The resulting apparent violations are of concern because
management expectations concerning configuration control were not well defined
and communicated to personnel, and management attention to address the
identified issues was inadequate to correct the problems.
