) HIGHLIGHTS OF WEEKLY INFORMATION REPORT WEEK END8NG AUGUST 17, 1990 I Meeting Between NRC Staff and NUMARC On Tuesday, August 14, 1990, there was a meeting between NRC staff and representatives of the Nuclear Managemeat and Resources Council (NUMARC) n l the seismic aspects of the Individual Plan Evaluation for External Events (IPEEE) program. NUMARC presented the proposed industry approach to the seismic part of the IPEEE. The main industry concern with the IPEEE, as put forth in Draft Generic Letter 88-20 Supplement 4 and Draft NUREG-1407, seems j to be the possible cost. NUMARC estimates a cost of $100M to collect the J l seismic information for the IPEEE. The NUMARC soproach uses a comparison of the seismic design bases of the plants and t1e probabilistic seismic hazards to subdivide the group of plants that should perform the full scope program into two groups: a full scope i review grous and a focused scope group would perform a somewhat reduced T1e staff agreed to consider the NUMARC proposal after the September program. Out of 72 participants, 18 were selected for program reviews and site visits by the NRC QA Team. The staff developed criteria for program and site evaluations which was tested on those 38 volunteers by the QA team. The information gained during the exercise will be used as the basis for developing licensing and inspection guidance for the = final rule. The 60-day trial period and the site visits were completed in mid July 1990. Beginning this week, the staff will conduct post-trial period work-shops to discuss ';he experiences, coments, and suggestions of the volunteers. E 1 o i t l Office of Nuclear Reactor Regulation Items of Interest l Week Ending August 17, 1990 l i Comanche Peak Unit 1 Commercial operation was declared at Comanche Peak, Unit 1 on August 13, 1990. 1 On July 13, the plant achieved 100% power for the first time. A 100-hour j warranty run began on July 17 and ended on July 22. The last major test of the l startup test program, a full trip from 100% power, was conducted on July 26, j A scheduled post-trip maintenance and repair outage ended on August 7, 1990. j l Fort Calhoun Station The original design of the Fort Calhoun Station only had two Auxiliary Feedwater l pumps. In order to increase its probability posture, the licensee incorporated a third Auxiliary Feedwater pump. This third pump was declared operable on August 11, 1990. The new diesel-driven Auxiliary Feedwater pump is nonsafety-related and will be used to provide the primary source of feedwater to the steam generators during plant startup, hot standby, and cooldown opera-tion. It will also provide a backup source of auxiliary feedwater to the steam generators. Nine Mile Point Units 1 and 2 Unit 1 On July 29, 1990, Unit I achieved criticality. The unit ha. been shut down since December 12, 1987, when a manual scram occurred and remained shut down under the provisions of a Confirmatory Action Letter issued in 1988, in response' to problems indicative of management ineffectiveness identified during the outage. Reactor coolant system leakage caused the unit to shut down hours af ter restart. i Misadjustment of the pilot valve on two electromagnetic Relief Valves (ERVs), and a broken spring on a vacuum breaker between the ERVs and the torus were found to be responsible for the increased reactor coolant system leakage. On August 2,1990, the unit was restarted after repairs to correct the leakage. On August 6, 1990, a manual reactor scram was initiated due to high vibration on a main turbine bearing. Prompt operator action to quickly reduce turbine speed limited damage to the bearing. A blank flange inadvertently left in the oil supply line to the bearing was the cause of the event. The lack of. i oil supply to the bearing was not detected earlier due to oil being supplied I i to the bearing by the turbine lift pumps. 1 On August 11, 1990, the unit was restarted following replacement of the bearing. On August 15, 1990, Unit I was operating at-22% power. The resident inspectors and region-based inspectors cor.tinue to monitor unit operations on a 24-hour basis. AUGUST 17, 1990 ENCLOSURE A

. - ~ l 1 11NP0 began a two-week site inspection of Units 1 and 2 on August 13, 1990. An ACRS ful' J mittee meeting is scheduled for September 6 or 7, 1990. Per Supplement. 2 to the Confirmatory Action Letter, the following activities i associated with the restart of Nine Mile Point Unit 1 are required: 1. Self assessment of operation during each power ascension plateau (25, 75, and 100 percent power). 2. Results of each self assessment to be presented to the NRC Resvart Assessment Panel, prior to increasing power to the next plateau and prior to routine full powar operation. Unit 2 The unit is at the 75% power level, coasting down to +he first refut ~Ing outage. South _ Texas Project. Units 1 and 2 Houston Lighting & Power Company (HL&P) announced yesterday that effective August 10, 1990 Mr. Mark R. Wisenburg will assume the )osition of Plant Manager, replacing Mr. Warren H. Kinsey (Daily Highlig1t of August 2, 1990,). Mr. Wisenburg's present assignment is General Manager, Assessment where he has responsibility for the safety committees, INP0 coordination, and planning. Prior to that he was Plant Superintendent, Unit 1. Mr. Wisenburg has been with HL&P since 1982. Waterford 3 ISLOCA Insper. tion Completed On August 10 1990, an exit meeting was held with the licensee of Waterford 3 to discuss the findings of the Interfacing System Loss of Coolant Accident (ISLOCA) inspection team led by DRIS/ SIB (Team Leader - J. Ball). The exit meeting was attendeo ty W. T. Russell, NRR, and J. Jaudon, RIV. The team found no single human error or equipment failure which could initiate an ISLOCA event at Waterford 3. However, a scenario was developed by the team which involved a % rmal transition from mode 4 to 5 that challenged two normally unseated-checkvolvesinthesuctionlinefromtherefuelingwaterstoragepool(RWSP) 'o the low pressure safety injection (LP51) pump when shutdown cooling was put in service. The results of this review will be used in developing a probabi-listic risk assessment of an ISLOCA occurring at Waterford 3 and will provide input into the overc l1 assessmer.t of the probability of an ISLOCA at a pres-surized water reactor facility. Yankee Rowe EDG Problems Last week, during the 18 month diesel generator surveillance testing, the licensees discovered that one of the EDGs enuld not supply the design rating (400 kw) after overhaul.. Subsequent testing of all three EDGs found that none could supply design rated load, in addition, it appears that the periodic testing does not demonstrate this capability even on an 18 month cycle because of the difference in conditions when testing in parallel with the grid as opposed to operating in isochronous mode (isolated from the grid). The licensee stated that the unit outage would be i extended a month to accomplish this replacement. Office of Nuclear Material Safety and Safeguards Items of Interest Week Ending August 17, 1990 International Atomic Energy Agency (IAEA) Inspection at the B&W Facility On August 10, 1990, an IAEA team initiated an announced inspection at the B&W Fuel Company in Lynchburg, Virginia. Safeguards Event Log Facility Analysis Report The 3afeguards Event Log Facility Analysis Report toc the first quarter of 1990 has been disseminated to regional and headquarters staff and the licensees. This issue of the report contains a number of innovations itended to provide improved feedback to the NRC and licensees. Of particular significance is the addition of a section which describes initiatives undertaki i by licensees to improve equipment operation or reduce human error. Survey of State and Tribal Emergency Response Capabilities for Radiological Transportation Incidents (NUREG/CR-5399) The survey report, prepared for the Division of Safeguards and Transportation by Indiana University, was published in May 1990. Since then, staff has distributed copies to interested parties, including survey participants in all 50 states, as well as 14 Indian tribes. Several hundred copies were i requested by the U.S. De)artment of Transportation and the Federal Emergency Management Agency for su) sequent distribution. Staff has also briefed the Federal Radiological Protection Coordinating Crm ittee and the Environmental Protection Agency's Inter-Agency Discussion Group on Indian Environmental Issues on the report. x State of Washington Petition On July 31, 1990, NRC docketed a petition for rulemaking from the State of Washington regarding the classification of Hanford high-level waste. The State originally submitted a petition to NRC on January 2, 1990, however, after consulting with NRC staff the State elected to revise the petition. The Office of Administration, Regulatory Publications Branch expects to publish a Notice of Receipt that will initiate a 60 day public comment period on this petition. 3 NRC Staff Observation of DOE /YMPO QA Surveillance of Lawrence Livermore National Laboratory From August 6-9, 1990, aDivisionofHigh-LevelWasteManagement(HLWM) I staff member observed a U.S. Department of Energy (DOE)/ Yucca Mountain Project Office (YMPO) surveillance of the Lawrence Livermore National Laboratory (LLNL) quality assurance (QA) program. LLNL is responsible for the development of the waste package for implacement in tuff, including definitions of the package environment, material development and testing, package design, performance analysis and testing. It was agreed by the i HLWM observer and the DOE /YMf 0 surveillance team that LLNL has an adequate QA program in place which is being effectively implemented. AUGUST 17, 1990 ENCLOSURE B

Items of Interest i Week Ending August 17, 1990 l Meetings with the Taiwan Atomic Energy Council l Dr. Anthony Tse (RES) visited Taiwan during July 1990 by invitation of the Taiwan Atomic Energy Council (AEC). The purpose of the visit was to discuss the NRC's rulemsking activities concerning the use of byproduct material in medicine. Dr. Tse met with the Taiwan AEC staff at four separate meetings and discussed the rulemaking on the quality assurance program for medical use licensees. The discussions also included the recent rulemaking on the preparation and therapeutic uses of radiopharmaceuticals as developed in l partial response to the petition submitted by the American College of Nuclear Physicians and the Society of Nuclear Medicine.. As arranged by the Taiwan AEC, Dr. 1se also made presentations on the same subjects to Taiwan's Society of ( Nuclear Medicine and Society of Radiology. -1 I o Office for Analysis and Evaluation of Operational Data Items of Interest Week Ending August 17, 1990 Division of Operational Assessment (DOA) The final exit briefing was held with Comonwealth Edison. Management on August 14 to discuss results of the Zion Diagnostic Evaluation. The team report 1 (including cover letter) and proposed staff actions were transmitted to the RES and NRR office directors and the RIII administrator on August 17. Concurrence was requested from NRR and RIII on the cover letter and comments were requested from all the above on the staff actions by August 22. On August 14, IRB staff met with members of the FRPCC to discuss revisions of the EPA PAGs. All remaining issues pertaining to the PAGs were resolved. EPA will incorporate agency comments in a new revision, to be available by the end of August. On August 16. IRB staff met with members of the FRPCC Federal Response Subcommittee to discuss a draft revision of the FRERP. The issue remaining is formalization of a health effects advisory group that could support yet not detract from the response on site. On August 16, the Director, AEOD, Acting Director DOA, and IRB staff met with representatives of the State of New Jersey and with the GPU licensee, to discuss the ERDS system and the possible involvement of New Jersey as a recipient of ERDS data. The Chief Specialized Technical Training Staff and Technical Program Specialist j met with Sandia National Laboratory personnel in Albuquerque, NM on August 14, 1990 to discuss the statement of work for Technology Transfer of Physical l Security System Vulnerabilities. The 'fTC is continuing work originated in 1988 by NMSS to support training for physical security personnel. APowerPlantEngineeringCourse(E-110)isbeinggiveninRegionIon August 20-31. This training is being given in response to a senior management high priority to support technical intern programs and will have attendees from Regions 1, III, and IV and NRR. This course is designed to provide students with an understanding of. prec';ical aspects of power plant operation. Emphasis is placed on the use and operation of various types'of equipment rather than on equipment design. This course, after some expansion, is seen as part of the l long range technical training solution to prepare interns for successful empletion of subsequent NRC qualification programs. Preliminary findings will be reviewed by Regional and headquarters offices and the results of the review will be confirmed in a letter to the State. Meeting with Oklahoma State Officialt i Dr. John Montgomery, Region IV Deputy Regional Administrator, and Charles. Hackney, Region IV State Liaison Officer, met with the following Oklahoma State officials in Oklahoma City, August 15, 1990, to discuss items of mutual interest: Dale McHard, the Governor-appointed State Liaison Officer for Oklahoma, Mark Coleman, Deputy Commissioner for Environmental Services, and H.A. Caves, Chief, Consumer Protection Services, State Department of Health. Topics discussed included LLW compact issues Agreement State status, NRC training for Stats employees, and the decommissioning and environmental surveillance of a number of fuel cycle facilities. The present system will be replaced with the same type card key system installed at One White Flint North (0WN). It is anticipated that installation will begin in late summer and be completed both cards in some buildings as components (readers) personnel may need to use by early Fall, 1990. During the installation phase are brought on-line/off-line. As with OWFN, the NRC photo identification badge will be used to activate the card reader and the green and white card will no longer be used when the upgraded system is installed. UPS Service Update .f The United Parcel Service (UPS) notified NRC that UPS employees, repre-t sented by the International Brotherhood of Teamsters, have accepted a nationwide, three-year contract. Routine UPS pickup and delivery service i to the NRC has resumed. 2. EXSIS Printing Feature Available for Event Information l t The Office of Information Resources Management has added a printing feature to EXSIS for event information. Individual Event Notifications r (ens) and Licensee Event Reports (LERs) can be printed on the PC printer after the event of interest is displayed on the monitor. The entire text of the EN will be prit.ted, as well as the complete abstract of the LER. Most printing obtained through EXSIS is only available on a screen by screen basis. The revisions are necessary to conform NRC regulations to the amended procedures contair.ed in the Federal Claims Collection Standards, as amended March 9,1984, and the Federal Claims Collection Act of 1966, as amended by l the Debt Collection Act cf 1982. The amended Part 15 regulations permit the NRC to further improve tne collection of debts due the United States. 10 CFR 16 " Salary Offset Procedures for Collecting Debts Owed by Fedaral Employees to the Federa. Government" The Debt Collection Act of 1982 and OMB Circular A-129 requires each agency to establish a salary offset program for collecting debts owed by Federal offset regulations to the Office of Personnel Management (OPM) posed salar employees to the Federal Government. A Notice of Violation and Proposed Imposition of Civil Penalty in the amount of $125 was issued on August 16, 1990 to Somat Engineeri_ng, Inc., Taylor,. Michigan. The action was based on failure to maintain constant control over a moisture density gai:ge. It' this -case, an employee made a { conscious decision to lesve the gauge unattended in.an unrestricted area for a short. time while he drove to a telephone to make a call..The base civil' penalty was partially mitigated because of the licensee's prompt: Identification and corrective action.- It should be noted that the-licensee-has not been specifically informed of the enforcement action. (EN 90-077)- l l 2. A Notice of Violation and Proposed Imposition of Civil Penalty in the amount of $5,000 was issued on August-16, 1990 to the U.S. Department of Agriculture, Washington, DC. The action was based on fifteen violations that, when viewed collectively, demonstrate a lack of. management oversight. Significant among these violations, there was a failure to insure that-l facilities under this broad license were inspected internally by the L radiation safety staff-at required. frequencies.- This violation was also i identified during NRC inspections in 1588 and'1989. '{ e Office of Consolidation Items of Interest Week Ending August 17, 1990 1 Second Building Status On August 13, 1990, the GSA contract negotiator met with WFN Managing Partner, Albert Abramson, to discuss WFN's concern about financial liabilities related to traffic mitigation at White Flint.that the Partners would retain if the- -Government chose to exercise its purchase option at the end of the fifth year of the lease of TWFN. The GSA negotiator believes a compromise resolution to this concern has been identified between him and Albert Abramson.. Some further checking up the GSA management chain is required however, to determine whether-the compromise 1: acceptable to the Government. On August 14, 1990, the Oft 1ce of Consolidation host J a meeting requested by' s the developer's architect, Karl Kaufman, to discuss the status of the NRC's base-building requirements for TWFN and various related issues.. GSA and the Offices of Administmtion and Information Resources Management also were represented at the meeting..Mr. Kaufman announced in the meeting that he expects to file er, application for an excavation permit on. September 1, 1990,. and to proceed on the schedule previously laid out by' Albert Abramrom which would result in receipt of an excavation permit by mid-November 1990. l We reported two weeks ago that the appraiser for TWFN had died unexpectedly the previous week and that GSA was reviewing bids for a new appraisal which might-delay the lease negotiations. GSA informed us en August 13 that.the deceased appraiser's company now says they intend to honor the existing contract and complete the appraisal on the previously agreed schedule, L 1.e., by August 26. l Traffic Mitigation On August 3, Senator Mikulski of Maryland introduced S. 2978, a bill te provide that Federal employees may participate in State-and local governn.ent programs encouraging the use of public transportation. The bill was referred to the Committee on Governmental Affairs. S. 2978 would authorize Federal' agencies to participate in programs such as the sale of discounted transit passes or'o'her i incentives that reduce the cost to employees of using public transportation, notwithstanding the provisions of Section 5536 of Title 5 of the United States Code which prohibits extra pay or allowances for employees whose pay is fixed by statute unless such extra pay is both authorized.by law and the-appropriation therefor specifically states that it is for the additional pay or allowance. Under existing law, the NRC is prohioited from matching the FARE-SHARE discount of 25 percent offered by Montgomery County on Metro, Ride-On and MARC tickets sold through the Energy Federal Credit Union branch at OWFN. i l o 1 j. Administrator and selected Region II staff were at the University of Virginia in Charlottesville, Virginia for site ~ familiarization and to present certificates to newly licensed reactor operators. 2. Carolina Power and Light Company A meeting was held with Carolina Power and Light Company management on August 15, 1990, to discuss a summary of their root cause analysis and. corrective action plans for their operator licensing requalification program. A detailed schedule of their plans was presented which will prepare them for en-April 1991 program reevaluation. 3. Virginia Electric and Powe'r Company On August 15, representatives of the Virginia Electric.and Powar Company were in the Regional Office to attend a management meeting regarding its Configuration Management Project. L ( 4. Tennessee Valley Authority i On August 16, the Regional Administrator and selected staff members, l participated in a management meeting at the Watts Bar site in Spring City, Tennessee, with the Senior Vice President Nuclear Power, regarding sche-duling activities at Watts Bar. l 5. Georgia Power Company 1 On August 17, selected staff members were-at the Vogtle Nuclear Station attending the exit meeting of tne Operational Safety Team. 6. Fraudulent Parts at Florida Power Light Compar;, FPL recently identified fraudulent valve packing material at one of their fossil plants. Because of.this, the licensee has reviewed valve packing material at both Turkey Point and St. Lucie nuclear sites. The licensee has discovered that some John. Crane Company packing material 1 at Turkey Point in fraudulent; howevcr, all of the suspect material is utilized in nonsafety-related applications. The review at the St. Lucie facility revealed no fraudulent packing material.'. The fraudulent materials were supplied by a local (Hollywood, Florida) distributor, Southern Mechanical, that stated that the materials were repackaged locally. A John crew 'j representative has inspected the materials and determined that the packing itself appears to be fraudulent. The. licensee is continuing the investi-L gation to determine if the distributor supplies other materials to the utility. . ~ - -0 0 7. McGuire Emergency Exercise On August 15, 1990, the licensee conducted a small scale. emergency exer-. cise (not observed by FEMA). The' licensee's performance was adequate, but one exercise weakness and one apparent violation were identified.' 8. Babcock and Wilcox Fuel Fabrication' Plant Region 11 recently became aware that the licensee has, apparently un-knowingly, been burying uranium contaminateo sludge onsite. The sludge is a result of treating waste water from the recycle and recirculation water systems. The recirculation system contains concentrations-of uranium below MPC. In the sludge'the levels of uranium averaged approximately 150 pC1/gm..The licensee has also notified the EPA that, because of^this finding, pond sediment previously thought only to contain cliemical waste-may also contain radioactive material.
