disclosed that the firs listed a single corporate officer.: During an NRC Region !V prelicensing visit to KI. the applicant reiterated that K! had only

.the single corporate officer.- A concurrent Office of Investigations. Region IV(01:RIV)investigationhadpreviouslyidentifiedthebrotheroftheKI principal corporate officer as the owner of a firm which had conducted

>redioactive well logging operations in Oklahoma for'2 years without an NRC i

itcense. 01:RIV.. inquiries. disclosed that K! had submitted.the names of the listed KI corporate officetr and his brother as officers of;KI to the Oklahoma

.l State Tax Commission, which conflicted with information previously provided to NRC:RIV in the license app,lication and during the prelicensing visit.-

L The 01:RIV investigation disclosed that the conflicting information was'the result of administrative error in the preparation of tu Oklahoma State Tax Cosmission forms and that KI made no false statements or false representations to the NRC.
