COMBINED INSPECTION REPORT NOS. 50-245/93-26, 50-336/93-22,

AND 50-423/93-21

This letter refers to the announced safety inspection conducted by Mr. J. Stewart and others

of this office from August 30 to September 3,1993. The inspectors reviewed the programs

being implemented in response to NRC Generic Letter 89-10, " Safety-Related Motor-

Operated Valve Testing and Surveillance." This generic letter and its supplements provided

guidelines to ensure motor-operated valves would be functional during design basis accident

conditions thereby protecting the health and safety of the public. At the conclusion of the

on-site inspection, the results were discussed with Mr. F. Dacimo and others of the Millstone

station. Additional information was obtained after the on-site inspection and the inspection

concluded on December 15,1993, with a review of the status of the findings as detailed in

the report.

During the inspection, your staff determined that the Millstone Unit 2 power operated relief

block valves would not isolate a design basis failure of the power operated relief valves.

This determination was made using preliminary data from EPRI sponsored valve testing.

Although the determination that the block valve capability was outside the design basis for

the system, the NRC is refraining from issuing a notice of violation because among other

items, your actions to evaluate the valve capability using preliminary data is considered

prudent.

This letter also refers to the presentation by your staff regarding your self-assessment of the

Northeast Utilities Motor-Operated Valve Program. 'niis presentation was held in the NRC

Region I office on September 28,1993, and the handout provided by your staff is included as

Enclosure 2 to this letter. We have found this assessment to be comprehensive and of high

quality. Weaknesses identified during the assessment were significant to the satisfactory

completion of the generic letter program and were being corrected at the time of our

inspection. We request your continued attention for the timely and effective disposition of

these weaknesses. We intend to review these corrective actions in future inspections.
